Hi Randy,
This is a follow up to our conversation today. DEQ does not have a record of the required Municipal
Separate Storm Sewer System (MS4) permit renewal application on file, which was due on August
23, 2021. The Town needs to submit a permit renewal application form immediately, which should
be signed by the Mayor or Town Manager. An electronic copy should be emailed to me, and an
original “wet” signature copy mailed to the address below. You will also need to submit the Town’s
current Stormwater Management Plan (SWMP) with the MS4 Permit Renewal Application form,
which can also be found on the NCDEQ-DEMLR Stormwater Program web page. Failure to submit
an application may result in issuance of a Notice of Violation for operating without a permit and
failure to reapply.
The Mayor or City/Town Manager may delegate another individual to sign permit documents using
the Stormwater Delegation of Signature Authority Form.
