Culp, Inc. Closed Loop Recycle Wastewater

Dear Mr. Creech:
On May 6, 2022, Division of Water Resources staff Caitlin Caudle conducted a routine
compliance inspection. A review of records, treatment facility, and monitoring wells were
conducted. Mark Creech, Derr Leonhardt, Tyler Miles, and Chad Cusin were present
during the review of records, facility, and monitoring wells. Chris Miles and Barry Bryant
were also present during the review of the facility and the monitoring wells. This review
reflected non-compliance with the subject permit. The following deficiency was noted:
1. Effective December 1, 2021, effluent sampling increased from yearly in November
to three times a year in March, July, and November, as listed in Attachment A.
Sampling of effluent was not completed in March 2022. This is a violation of the
subject permit. Moving forward please ensure that all sampling is conducted
at the required frequency.
The following item of concern was noted:
A. Per Permit Condition 111.7 "a protective vegetative cover shall be established and
maintained on all berms, pipe runs, erosion control areas, surface water diversions,
and earth embankments...." At the time of inspection, little living vegetation was
present on the earthen berm surrounding the concreate containment structure.
Please continue to make efforts to establish a vegetative cover on this earthen
berm.

Reason for Visit: Routine
Inspection Summary:

Other observations:
• Stormwater is now being pumped form the earthen berm to inside the concrete containment area so any effluent
captured in the earthen berm can be retreated. The earthen berm is serving as a secondary containment area.
• Ms. Caudle requested that at least the emergency contact information from the spill plan be posted in the treatment
building. A central point of contact handles any spills.
• A lock was present on the valve to release stormwater from the earthen berm.
• Rip rap was present under the stormwater release valve.
• Locks were present on all monitoring wells. All monitoring wells were in good condition.
• Shamrock still hauls residuals captured from the dewatering roll off box and wastewater from the reverse osmosis waste
tank. The 8,000 gallon "clean water tank" hold wastewater from the reverse osmosis process.
• Shamrock hauls residuals once every three weeks.
• The 3,650 gallon pump tank with high-water alarms and two Gorman -Rupp trash pumps with float level switch for large
solids removal is no longer present.
• Maintenance records are maintained as Preventative Maintenance logs that are completed monthly.
• Inspection logs are completed daily.
