6. Comments: Inspection conducted during MS4 stormwater permit audit by the State of
North Carolina. Multiple items for correction, see below.
7. Corrective Actions: Re -inspection will occur in 7 days to ensure items are repaired.
A- Rear perimeter silt fence/silt fence outlets need cleaning, maintenance, fresh stone, and/or
repairs. Some outlets on East portion of site were recently refreshed. Silt fence outlets should
have washed stone on the hardware cloth to filter the water, straw hay bales should not be
used. Several areas of off -site sediment on the East and West ends of the site have occurred
into the buffer and directly in the creek. These should have been caught on your self
inspections and reported to the State DEQ/DEMLR staff. Coordinate clean-up of lost
sediment into the buffer and creek using low impact methods.
B- FES 3 on the East side of site needs to have rip rap pad and plunge pool installed per the
approved plan. What's in place has eroded out. Ensure pipe/FES were installed at the right
elevation. Slope is real steep and not ideal for proper rip rap dissipater and plunge pool
installation.
C- Drop inlet protection needs to be in place on all inlets installed. Some are missing or full of
sediment and needing cleaning out with fresh stone.
D- Diversion ditches going into basins need to be cleaned out and stabilized to maintain positive
flow into basins.

E- Ground cover needed immediately on East portion of the project where no grading activity
currently in place.
F- Both sediment basins need to be mucked out and baffled chambers restored so they can hold
designed capacities.
G- Rain gauge on inspection box was broken and self inspection reports were incomplete. More
info besides rain totals should be recorded in those.
