Per our recent discussion concerning this operator, please find attached his certification card
(No. 6010) that we received August 4, 2003.
Due to a number of factors, Mr. Steedly felt that now is the appropriate time to surrender his
certification and retire from being an ORC.
Based on the attached letter, it is the recommendation of the FRO that the Commission accept
his certification with the understanding that he will not be seeking reinstatement in future.
I would like to recognize Don Register and Hughie White in working on this issue. Their
continued efforts to bring the Bay Tree wastewater collection and treatment facilities into
compliance are greatly appreciated.

Dear Mr. Jones:
The Technical Assistance and Certification unit received notification from the Fayetteville
Regional Office of DWQ that the ORC listed on the DMR reports is not the same as listed in our
files and it also appears that a Back -Up ORC has never been designated.
Please be advised that, according to T15A: 08G.0201.2 andIT15A: 08G.0202.2(B) you
are required to designate a ORC/Back-up ORC of the same type and, grade of the facility for the
ORC and a Back -Up operator -of the same type and no more than one grade lower than that of
the facility. as system owner you are required to have read and understood the rules as they
apply to you (T1 5A:0202(a)(1).
Therefore, you are required to fill the position of Back -Up ORC for your facility
with a qualified operator within sixty days and as set'forth above. Failure to designate will
result in a Notice of Violation.
