Wastewater Irrigation System Permit No. WQ0018708 County - Bladen Dear Mi. Jones: Donald R. van der_ Vaart Secretary You are hereby notified that, having been issued a permit for spray irrigation operations for the subject facility, Lake Creek Corporation, was found to be in violation) of the following permit conditions. Violation I: Failure to meet the effluent limits of Attachment "A"for Permit Number WQ0018708. Review of the monthly reports for Lake Creek Corporation., Permit WQ0018708, (May 2012 - December 2014) revealed a violation for Flow, in conduit or through the plant. The limit for Flow is 20,000 GPD monthly average. (See Permit dated February 3, 2009) The facility has exceeded this maximum value each month for the period previously stated., , Suggested Corrective action forViolation • 1. Upgrade the facility to accommodate the increased flow, per Permit Section I.(1). 2. Eliminate Inflow and Infiltration concerns in the collection system. Stephen Jones Page 2 March 12, 2015 On November 12, 2014 during a compliance inspection, Tony Honeycutt of the Fayetteville Regional inquired about the absence of the facility's winter crop. It was communicated to Mr. Honeycutt by the operator (Tony Baldwin) that the seed for a winter crop had been broadcast over the site. Mr. Baldwin also called the Fayetteville Regional Office on December 8, 2014 stating that seed for a winter crop had been broadcast over the spray irrigation site again. On, March 2, 2015 a follow-up site visit was conducted by Mr. Honeycutt and no evidence of a suitable winter crop was found. Required Corrective Action for Violation II: If you have not already done so, immediately take actions to establish a suitable seasonal crop. It is recommended that you consult with the local extension office or an agronomist to assist you with this issue. The Division of Water Resources requests that a formal Plan of Action addressing the above violation be submitted, on or before April 15, 2015. Please be advised that this Notice of Violation does not prevent the Division of Water Resources from taking enforcement actions for this violation or any past or future violation. Neither does this notice remove you from the responsibility or liability for failure to comply with any State Rule, State Statue or permitting requirement.
