January 17, 2017. However, additional information is required before the reviews may be completed.
Please address the items on the attached pages no later than the close of business on April 20, 2017.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and
regulations. Any oversights that occurred in the reviews of the subject application packages are still the
Applicant's responsibility. In addition, any omissions made in responding] to the appropriate items
outstanding in Section A, or failure to provide the additional information on or before the above requested
date, may result in your applications being returned as incomplete.

A. Annual Permit Fees:
The annual fees for maintaining this permit are not up-to-date. Annual fees must be paid in
accordance with Regulation 15A NCAC 02T .0105. Annual fees are owed for the past four years
under Invoices #2013PR006260, #2014PR006464, #2015PR006474 and #2016PR006506. Each
of these invoices is for $1,310.00 and they represent a'total of $5,240 owed. These invoices must
be paid before a permit can be issued.
B. Ownership of the Facility:
The Bladen County GIS website indicates that the treatment works are located on property owned
by the Red Bird Land Company LLC. Under Regulation 15A NCAC 02T .1004(d), a permit can
only be issued to an entity which can provide:

(1) Legal documentation of ownership (i.e., a contract, deed, or article ,of incorporation);
(2) A written and notarized Intent to Purchase Agreement signed by both parties, accompanied by
a plat or survey map; or
(3) A written and notarized Lease Agreement signed by both parties,1 specifically indicating the
intended use of the property, as well as a plat or survey map. The Lease Agreement must adhere
to the requirements of Regulation 15A NCAC 02L .0107(f).
Please submit one of the following:
(1) Legal documentation of ownership by the Lake Creek Corporation (i.e., a contract, deed, or
article of incorporation);
(2) A written and notarized Intent to Purchase Agreement signed by both parties, accompanied by
a plat or survey map; or
(3) A written and notarized Lease Agreement signed by both parties, specifically indicating the
intended use of the property, as well as a plat or survey map. The Lease Agreement must adhere
to the requirements of Regulation 15A NCAC 02L .0107(f). ' •
C. Restrictions in the Renewed Permit:
The renewed permit will include a no -tap moratorium which shall be in place until upgrades are made to the
Baytree Lakes development collection and treatment system to allow greater flows into the system of up to
30,000 gallons per day. This restriction shall also limit any expansion requests to existing homes or structures
served by the Baytree collections and treatment system.

D. Man of the Sewer System:
A map prepared and sealed by a North Carolina Professional Engineer or North Carolina Professional Land
Surveyor is required which shows all existing collection lines (gravity and force main), pump stations,
manholes, etc. for all infrastructure installed to date.

E. Engineers' Certifications for all Infrastructure:
Engineers' Certifications for all infrastructure installed and in use is required. These Certifications include a
final (completion) certification for the 9,530 feet of gravity and force main sewer extension permitted in the
permit that was issued on December 12, 2005 and for which partial certification was issued in April of 2007.
Please submit Engineers' Certifications for all infrastructure installed and in use. Final certifications must be
submitted for those infrastructure components that have been certified in part and have since been completed.

Office of the County Manager
Bladen County operates under a Board of Commissioners/County Manager
form of government. The County Manager's Office oversees all County
departments, programs and operations pursuant to 'the policies, ordinances
and directives adopted by the Board of County Commissioners. Other
responsibilities include.preparing the recommended annual County budget,
preparing.and publishing the agenda for Board meetings, and providing all
other management services necessary to assure the efficient and effective
operation of County government.
The 'Clerk to the Board serves as corporate secretary to the Board,
custodian of the minute book and County seal, and certifies all official
documents on behalf of the County. The Clerk prepares the Board's
meeting agenda, attends all meetings of the Board, and records and
transcribes the official minutes of meetings.
The County Attorney is appointed by the Board of Commissioners and
provides advice and counsel to ensure government activities are carried out
in a judicious manner. .

Recommendations
1. Water service will be available at the street right of way adjacent to your lot. You
should request the county's service to be located on your property 30 ft from the
center of the highway, across any ditch off state right of way. You may determine
this location of request a plumber to advise.you.
-2. Prior to making the final connections to the new system, the owner should drain
their hot water heater and flush. Also, the entire system should be flushed by
opening all faucets and allowing it to run until the water becomes clear. Because of
the new source of water and possible Increase of pressure, deposits in. your pipes
may be loosened and will need to be flushed. -
3. Residents may use their existing pumps as long as it is not connected to the
County System.
4. The owner, or a plumber, should visually inspect the entire plumbing system and
check for leaking pipes and faucets. -
Highlights of NC State Plumbing Code Requirements
1. A minimum of 160 psi 3/4" service line from the meter to the structure.
2. An approved cut-off gate valve must be installed near the structure and at the
meter box.
3. Service line must be buried a minimum of 12" in the ground.
4. Service line may not be installed through a septic tank drain field or within 10 ft.
of a septic tank.
5. No private water supply shall be interconnected with,the public water supply.

The purposes for which Bay Tree Lakes Property Owner's Association is organized are to provide for the preservation of the values and amenities of
Bay Tree Lakes, and to own, control, operate, improve, and maintain the common properties within said community in accordance with the provisions
of a Declarationof Covenants and Restrictions.
The role of the Property Owner's Board of Directors is to coordinate, provide direction, and facilitate the'above.
POA Monthly Board Meetings:
Your Board of Directors strongly encourages property owner attendance at our monthly board meetings. We have some very serious issues to manage
our way through, and we believe it is vital to have the support of the property owners. All meetings are held at the Clubhouse on the second Thursday
of the month, starting at 7:OOp.m. All are welcome.

Daniel Stephen Jones, .91\of 3i4 West Main Street, died peacefully at home on
Wednesday, October 1.2th, of:natural causes_. • • •
A Graveside Service will be held on Monday, October 17th, at l3: oo a:m. in the Clinton
Cemetery with Dr: Jim Moran otciating_
Known as Steve, Mr. Jones was born in Wagram, Scotland County, North Carolina. He
moved to Clinton as a child and was graduated from Clinton High School_ He served his
country in World War II in the Army Air Corps as a bomber pilot. Upon returning from
service, he enrolled in the University of North Carolina at Chapel Hill where he earned a
B.S. in business and accounting and a Doctorate in jnris prudence.
Mr. Jones practiced law in Clinton for over 45 years, served as Sampson County
Attorney, served on the Sampson Memorial Hospital Board, the Board of Harrells
Christian Academy, and supported numerous other civic organizations and causes in
Clinton and the surrounding the area, an area of our state about which cared greatly.
For many years Mr. Jones ran a farming operation in Sampson County and was the
principal developer of the Bay Tree Lakes in Bladen County, a community about which ,
he was passionate and determined. A member of Graves -Memorial Presbyterian ' `. " '
—Church-in Clinton -since -childhood, he served the church as deacon and in other
capacities: While Mr_ Jones was accomplished in the various aspects of his business and
personal life, he may be: best remembered •for his unfailing kindness•and his respect for
He is survived by three children, his daughters, Hendry Purcell Jones of Richmond,
Virginia; and Ruth James Atkins Jones Gebhart and husband, Roger, of Charlotte; and
his son, Daniel Stephen Jones, Jr., and wife, Becca, of Charlotte; six grandchildren,
James Edward Bair Gebhart, Roger Stephen Davis Gebhart, Rebecca Allston Jones,
Daniel Stephen Jones, HI, James Atkins Bates Jones and Ruth Hendry Jones, all of
Charlotte. He is also survived by his beloved nieces and nephews and his caregiver and
great friend of ten years, Mrs. Mary Sampson of Clinton_
He is predeceased by his parents, Rowland Judson Jones and Eliza Purcell Alderman
Jones Lesley, also of Clinton; his wife, Ruth James Atkins Jones of Clinton; his brothers,
Rowland Judson Jones, Jr. and John Purcell Jones, both of Morehead City.
