However, additional information is required before the reviews may be completed. Please address the items on the attached pages no later than the close of business on April 20, 2017. Annual Permit Fees: The annual fees for maintaining this permit are not up-to-date. Annual fees must be paid in accordance with Regulation 15A NCAC 02T .0105. Annual fees are owed for the past four years under Invoices #2013PR006260, #2014PR006464, #2015PR006474 and #2016PR006506. Each of these invoices is for $1,310.00 and they represent a'total of $5,240 owed. These invoices must be paid before a permit can be issued. B. Ownership of the Facility: The Bladen County GIS website indicates that the treatment works are located on property owned by the Red Bird Land Company LLC. Under Regulation 15A NCAC 02T .1004(d), a permit can only be issued to an entity which can provide: •y (1) Legal documentation of ownership (i.e., a contract, deed, or article ,of incorporation); (2) A written and notarized Intent to Purchase Agreement signed by both parties, accompanied by a plat or survey map; or (3) A written and notarized Lease Agreement signed by both parties,1 specifically indicating the intended use of the property, as well as a plat or survey map. The Lease Agreement must adhere to the requirements of Regulation 15A NCAC 02L .0107(f). Please submit one of the following: (1) Legal documentation of ownership by the Lake Creek Corporation (i.e., a contract, deed, or article of incorporation); (2) A written and notarized Intent to Purchase Agreement signed by both parties, accompanied by a plat or survey map; or (3) A written and notarized Lease Agreement signed by both parties, specifically indicating the intended use of the property, as well as a plat or survey map. The Lease Agreement must adhere to the requirements of Regulation 15A NCAC 02L .0107(f). ' • C. Restrictions in the Renewed Permit: The renewed permit will include a no -tap moratorium which shall be in place until upgrades are made to the Baytree Lakes development collection and treatment system to allow greater flows into the system of up to 30,000 gallons per day. Man of the Sewer System: A map prepared and sealed by a North Carolina Professional Engineer or North Carolina Professional Land Surveyor is required which shows all existing collection lines (gravity and force main), pump stations, manholes, etc. for all infrastructure installed to date. He moved to Clinton as a child and was graduated from Clinton High School_ He served his country in World War II in the Army Air Corps as a bomber pilot. Upon returning from service, he enrolled in the University of North Carolina at Chapel Hill where he earned a B.S. in business and accounting and a Doctorate in jnris prudence. Mr. Jones practiced law in Clinton for over 45 years, served as Sampson County Attorney, served on the Sampson Memorial Hospital Board, the Board of Harrells Christian Academy, and supported numerous other civic organizations and causes in Clinton and the surrounding the area, an area of our state about which cared greatly. For many years Mr. Jones ran a farming operation in Sampson County and was the principal developer of the Bay Tree Lakes in Bladen County, a community about which , he was passionate and determined. " ' —Church-in Clinton -since -childhood, he served the church as deacon and in other capacities: While Mr_ Jones was accomplished in the various aspects of his business and personal life, he may be: best remembered •for his unfailing kindness•and his respect for He is survived by three children, his daughters, Hendry Purcell Jones of Richmond, Virginia; and Ruth James Atkins Jones Gebhart and husband, Roger, of Charlotte; and his son, Daniel Stephen Jones, Jr., and wife, Becca, of Charlotte; six grandchildren, James Edward Bair Gebhart, Roger Stephen Davis Gebhart, Rebecca Allston Jones, Daniel Stephen Jones, HI, James Atkins Bates Jones and Ruth Hendry Jones, all of Charlotte.
