Only the front page of DMR uploaded; the signed page was not included. Also, the COC number was not noted on the
DMR (only the General Permit NCG030000 entered instead). There is no need to submit qualitative monitoring results
(those should be kept on site with records). Please resubmit a complete copy of the DMR with signature.
