form to this office at the address listed below. Please be certain to include owner and
agent information, owner and agent signatures, and dates the form is signed Attached is
an example form that you may use if you wish
2) Please note that the Stream Classification for the Unnamed Tributary (UT) to the Neuse
River associated with your project is WS -IV, NSW (index # 27 -(20 7)) rather than C; NSW
Additional information is not needed at this time, but this change should be noted as water
quality standards, use classifications, and development requirements are different for C
and WS -IV waters
Please submit this information within thirty (30) calendar days of the date of this letter If this
information is not received within 30 days, DWR will not process the application, and will return it
