Dear Mr. Lynch:
On March 9, 2022, Cheng Zhang, and Alys Hannum with the Raleigh Regional Office (RRO) of the
Division of Water Resources (DWR), conducted a compliance sampling inspection of the subject
facility. The assistance of Ms. Britany York, the WWTP Superintendent and Operator in Responsible
Charge (ORC), and Ms. Bobbie Hill, the backup ORC and laboratory supervisor,was greatly
appreciated during the inspection. Observations during the inspection are summarized below, as well as
an inspection checklist is attached for your records.
Siler City WWTP is permitted to discharge at a rate of 4.0 MGD and consists of: automatic and manual
bar screens, grit collection unit, influent pump station, influent equalization basin(Zone 2), dual
oxidation ditches with surface jet aeration, flow splitter box, alum feed station, lime feed station, dual
secondary clarifiers, dual aerobic digesters, sludge transfer station, dissolved air flotation(DAF) unit
(Used as needed), sludge thickener basin, influent equalization or sludge storage basins (Zone 3 A & B
used as needed), four tertiary filters, filter backwash basin, gaseous chlorine disinfection, chlorine
contact chamber, gaseous sulfur dioxide dechlorination, and step-aeration. The facility discharges into
Loves Creek, which is classified C waters in the Cape Fear Basin.
1. The current permit was modified on April 30, 2019 and expired on May 31, 2019. A draft
permit was published on March 31, 2022 for public comments.
2. Basinwide Information Management System(BIMS) classifies the facility as a WW-4
Wastewater Treatment Plant. Britany York is listed as the Operator in Responsible Charge
(ORC). Bobbie Hill and Chris McCorquodale are listed as Back-up Operator in Responsible
Charge (Back-up ORC).
3. At the time of inspection, all listed treatment units were in operable conditions with a few
exceptions: the DAF unit has not been used in the past years; EQ basin 4A was not used to hold
wasted sludge; one recirculation pump at oxidation ditch#1 was taken offline for repair. Wasted

activated sludge was treated in the two aerobic digesters and two EQ basins, and thickened in the
sludge thickener basin. A portable belt press unit (rented from Synagro) was in operation on site
to dewater sludge from the thickening basins. The inspectors noted the following:
• Dissolved oxygen (DO) level in both oxidation ditches was lower than needed: 0.92 mg/L
and 0.34 mg/L in ditch #1; 0.67 mg/L and 0.24 mg/L in ditch #2 (readings from four
inline DO meters). A floating aerator was in operation at Ditch #1. Mix liquor suspended
solids was 5800 mg/L (March 8, 2022).
• Sludge blankets at two secondary clarifiers were high, 8 feet for clarifier #1 and 9 feet for
clarifier #2.
• EQ basins 3A and 3B were full with sludge; Digester A and B both had about 3 feet of
freeboard. EQ basin 4A was empty. Two sludge thickening basins had about 1/4 space
left.
4. Records Review: Discharge Monitoring Report records for January 2022 and July of 2021 were
reviewed with lab bench sheets and no transcription errors were noted. Chain of Custody
forms are completed and kept on site. A daily operation log book is maintained and kept on site.
Records are maintained for at least three years.
5. The laboratory on site is certified to analyze conventional parameters (Lab Certification No.
132). Toxicity and metals are sent to a contract lab for analysis. A cursory review of the
laboratory noted all pH buffer stock was within expiration dates. Field meter calibration were
performed as required. The sample refrigerator, as well as the fecal and BOD incubators, were all
found to be compliant with temperature requirements.
6. The right of way to the discharged point is maintained. The effluent was clear and free of foam.
No visible impacts were noted immediately downstream of the discharge point.
7. As part of the CSI, influent and effluent composite samples collected using the facility's samplers
were split into two portions and analyzed for permit required parameters by Siler City WWTP
laboratory, NC DWR laboratory, and a contract laboratory (Meritech, Inc.). The inspector also
collected a grab effluent sample for analysis of fecal coliform at DWR laboratory. Field
parameters were analyzed by facility staff.

Sampling results show that effluent BOD, ammonia nitrogen, and Fecal coliform were higher than the
weekly average limits; high TKN and low nitrate/nitrite indicate poor nitrification.
At the time of inspection, the treatment plant was recovering from an upset happened in previous
months. The treatment plant upset started in January 2022 caused by equipment failure (blowers to the
oxidation ditches, and tertiary filters). During the process to repair the blowers, aeration in the oxidation
ditches were reduced or even turned off, resulting in breakdown of biological treatment process. Tertiary
filters were also taken offline for unscheduled maintenance. Situation got worse in early February due to
flow from the Town's Water Treatment Plant (WTP) and Mountaire Farm. Flow from the WTP (mixture
of backwash water and sludge) contained high level of iron (Facility staff observed high concentration
of iron (375 mg/L) in influent on March 6, 2022). Mountaire Farm failed to notify the facility staff of
chemical change (switch to peroxide from bleach). The combination of flow from the WTP and
Mountaire Farm caused complete die -off of microorganisms in the oxidation ditches and mixed liquor
turned septic. Facility staff managed to recover from the upset by wasting toxic sludge out of the system
and introducing manufactured microorganisms. The recovery process was interrupted in mid of March
by two high flow events during which the facility staff had to turn aeration off to prevent bulking of the
clarifiers or washout of biosolids from the oxidation ditches.
Facility staff stated that a portion of the jet aerators at the oxidation ditches were not functioning and
they would be able to take either ditches offline for necessary repair only after a new oxidation ditch is
constructed as part of the plant upgrade plan. The floating aerator added to ditch #1 helps improve the
aeration to a certain extent, the facility may need to add more floating aerators to the oxidation
ditches before being able to repair the inoperable jet aerators.
Town of Siler City was found to have violated Part II, Section C, 2 of its NPDES permit for failure
to proper operate and maintain the wastewater treatment plant by failure to provide sufficient
aeration for the biological treatment process.
A Notice of Violation/Intent to Issue Civil Penalty is being issued for the noted violation of North Carolina
General Statute (G.S.) 143 215.1 and the facility's NPDES Permit. Pursuant to G.S. 143 215.6A, a civil
penalty of not more than twenty-five thousand dollars ($25,000.00) may be assessed against any person who
violates or fails to act in accordance with the terms, conditions, or requirements of any permit issued
pursuant to G.S. 143 215.1.

within the 10-day period, a civil penalty assessment may be prepared.
Remedial actions should have already been taken to correct this problem and prevent further occurrences in
the future. The Division of Water Resources may pursue enforcement action for this and any additional
violations of State law. If the violations are of a continuing nature, not related to operation and/or
maintenance problems, and you anticipate remedial construction activities, then you may wish to consider
applying for a Special Order by Consent.

Facility staff stated that the plant was upset by slug load of sludge from the Town's water treatment
plant through the Town's sewer collection system in February 2022. Facility staff observed high
concentration of iron (375 mg/L) in influent on March 6, 2022, which indicated dumping of sludge
from the water plant.
The facility also experienced blower issues in February 2022 resulting in periods without aeration at
the oxidation ditches. At the time of inspection, all blowers were operational, however, DO level in the
ditches was lower than needed. Facility staff stated that some of the jet aerators were not functioning
and they could not aford to take either ditches offline for repair. The floating aerator in Ditch #1
helped to a certain extent. Before the oxidation ditches can be taken offline for repair, perhaps more
floating aerators are needed to provide sufficient aeartion in the ditches.
