Dear Ms. Hamilton:
Mr. Ron Boone, of the NC Division of Water Resources (NCDWR), Winston-Salem Regional
Office (WSRO), conducted a compliance evaluation inspection of the Woodlake mobile home
park wastewater treatment plant, on March 23, 2022. Mr. Boone met with Randy Bell, Operator
in Responsible Charge (ORC), and Bradley Flynt, Backup ORC. Their assistance and cooperation
were greatly appreciated. Inspection findings are detailed in the attached Water Compliance
Inspection Report and summarized below.
- The comminutor is not listed in the permit's plant description.
- The aeration header may be too small.
- There is no spare pump on hand for the influent pump station.
- The pump station's telemetry system should be evaluated as it did not call the ORC's cell
phone, as it was purportedly programmed to do, the first several times the telemetry system
was triggered/tested.
- The pump station's audible/visual alarms did not operate properly during the inspection.
This must be repaired within 30 days of reception of this inspection report.
- The flow meter is due for calibration in March, 2022.
- The backup ORC is performing more visitation than he is allowed to do by regulation. This
needs to be adjusted or the backup ORC designated as the ORC.
Please be aware that the Division may take enforcement action against any permittee who fails to
comply with any requirement of their permit. Pursuant to NC General Statute (NCGS) 143-215.6A,
a civil penalty of not more than twenty-five thousand dollars ($25,000.00) may be assessed against
any person who violates or fails to act in accordance with the terms, conditions, or requirements
of any permit issued pursuant to G.S. 143-215.1.

Ron Boone, of the NC Division of Water Resources (DWR), Winston-Salem Regional Office, met with
Randy Bell, Operator in Responsible Charge, and Bradley Flynt, Backup Operator in Responsible
Charge, on 3/23/2022, to perform a compliance evaluation inspection of the Woodlake wastewater
treatment plant. Below is a summary of the inspection findings.
- The comminutor is not listed in the plant description area of the permit.
- The operators stated the aeration header was recently replaced with too small a pipe, which can
make it difficult to maintain sufficient dissolve oxygen levels in the aeration basins. Yes should
evaluate upsizing the aeration header to better maintain dissolved oxygen levels.
- There is no spare pump on hand for the influent pump station.
- The pump station's telemetry system was not calling the ORC's phone as it was purportedly
programmed to do. It did eventually call his phone, near the end of the inspection, which is unusual.
Yes should have the system evaluated to eliminate bugs that may be causing the system not to be
able to call the ORC's phone for some reason.
- The pump station's audible and visual alarms were not operating properly at the time of the
inspection.
- The flow meter is due for calibration in March, 2022.
- The backup ORC (Bradley Flynt) is performing more often than allowed (40%) by regulation. This
needs to be corrected or Mr. Flynt needs to be reassigned as the ORC.
- All reviewed documentation was compliant and all required documentation was readily available,
complete, and current.
Mr. Boone noted no other concerns, deficiencies, or violations.

Comment: According to eDMRs for January through March 2022, Backup operator, Bradley Flynt,
conducted visitation on 65% of the days the ORC was required to visit in January and
February, and on 70% of the days the ORC was required to visit in March, which
significantly exceeds the 40% of visits the Backup ORC is allowed to conduct. The
ORC, Randy Bell, must complete no less than 60 percent of the required visitations.
