The Traffic Control Coordinator will be Andrew Fulkerson. The Certified Erosion Control Supervisor will be TBD. Project Schedule/Contractor Schedule: Date of Availability: March 21 st, 2022 Completion Date: 180 days after the completion of ICT # 1 The Contractors Plan of Operation Start Dates: TBD — Late August/Early September Contract/Plan Review: Liquidated Damages are $200 per day. ICT # 1: Liquidated damages are $700 per day. ICT #2: Liquidated damages are $1,500 per day. DBE goal of 3.0% was met. The contractor was reminded that materials cannot be used in the work until they have been approved, tested, or certified. ERIC BOYETTE SECRETARY Utility Comments: • Utilities are complete with the exception of one pole remaining to be removed within the next couple of weeks. Right of Way Comments: • No R.O.W. issues. General Comments: • Make sure if you have issues meeting commitments, document all communication early and often, so that if you fail to meet the committed amounts, we can go through the good faith effort to replace. • If you plan to use a drone (follow guidelines) • Please review the idle equipment guidelines in the contract • Make sure tracks on equipment are clean upon arrival to the project and get proper permits if it crosses state lines (Pest/Plant Quarantine) • Make sure that a Level II certified person is always onsite to assist with erosion control implementation • Our inspectors will fill out the NPDES forms, make sure your level II person does the inspection as well and reviews the NPDES afterwards. • The NPDES forms are to be kept onsite at all times with a set of up to date as - built plans. • Stage seed early and often to help achieve 80% vegetation after work has been completed • Seed changes to make — Add hard fescue to native seeding mix (recommend 20lbs per acre) • Someone from roadside environmental will be performing a monthly inspection and any corrections will be provided to inspector to communicate said changes to contractor. • *Please make sure that corrections provided are addressed in a timely manner and not left to be an issue for next month's inspection. • This contract has a lawn type appearance special provision. ERIC BOYETTE SECRETARY • Concrete washouts will be required and will need to be built per the spec (bags are acceptable) • Make sure to follow submittal requirements in the contracts whereas the proper units listed, Geotech and structures, are emailed in a timely manner to provide sufficient time to review. (Please CC David and Nathan so that we can track the progress) • Make sure that anyone operating a crane is certified to do so and has their card present during operating. Please provide the inspector onsite with a copy so that they can record it in Shareplus. • Please make note that creek water will not be allowed to be used in the production of grout. Water for miscellaneous uses will need to be from an approved source. • Asbestos assessment has already been completed for this project. • Please inform project inspector of guardrail and pipe deliveries prior to them getting onsite so that they can have our M&T rep inspect necessary materials before they are installed. You do not want to dig up pipe for inspection. • Please let the inspector onsite know of any anticipated night or weekend work so that we can staff the project properly. • We need to coordinate with Camp Daniel Boone when they have events to limit traffic jams and backups. • Please note that if areas are used as a staging area, we would need a reclamation plan if any ground disturbing activity takes place in that area off of right-of-way. • Pond already drained. • Kevin Mitchell to do monthly inspections. • Self report any sediment loss or oil spills. • Do not use earth material to construct Impervious Dikes (if needed) • Inside limits of project and SHIPPO — contractor to park equipment on property owners property if permitted. • Less traffic from September on to Camp Daniel Boone. • Phase I — SA for temporary signals instead of using the stop/yield TCP. • Very little weekend traffic on driveway that's getting relocated. • Keep traffic in roadway work completed in Phase I for Phase II traffic pattern.
