However, the compliance action does not stay any permit condition. Part V, Paragraph 11 of the permit requires submittal of the permit renewal application 180 days prior to expiration. On May 25, 2021, a reminder to apply for renewal was sent to the permit contact on file (see attached). At this time, you just need to submit the short MS4 application form. Hi Jeanette, I don't see where we received comments back on the SWMP that was submitted last spring. Do you have any feedback on that? Also, we submitted the annual report last August permit so I'm a little confused on the timing. Can we set up a call to go over what we need to do and the status of DEQ's review of the SWMP that was submitted last year? Every MS4 permit contains a condition requiring that a permit renewal application be submitted at least 180 days prior to expiration. Having an MS4 permit renewal application on file automatically authorizes the MS4 to continue operating under the expired permit until a new permit is issued. If no renewal application is on file, then the MS4 is operating without a permit. If you feel you have received this notification in error, please just let me know and we can work to get the error resolved. If you need to submit a permit renewal application, you only need to submit two items: (1) the short MS4 application form and (2) your current Stormwater Management Plan (SWMP) if one has not already been submitted in response to an audit/compliance notice. If there is no Designation of Signature Authority (DOSA) form on file, then the city/town manager or mayor must sign the application form. To submit a DOSA, complete the Permit Contact Update Request Form. The city/town manager or mayor must sign the DOSA form. You can check your permit file online in the Stormwater Program Laserfiche Repository to see what documents are currently on file. Please let me know if you need any assistance.
