POST OFFICE BOX 709 BEULAVILLE, NORTH CAROLINA 28518 Dear Mr. Smith: NORTH CAROLINA Environmental Quality April 18, 2022 Subject: Permit No. WQ0030998 Triple S Farms and Cleaning Class B Residuals Program Attachments A and B Certification Duplin County In accordance with your permit major modification request received January 6, 2022, and subsequent additional information received April 1, 2022, we are forwarding herewith modified Attachments A and B certified April 18, 2022, to Triple S Farms and Cleaning, Inc. for the subject residuals management program. Modifications to the subject Attachments are as follows: ➢ Addition of the Town of Maysville WWTP (Permit No. NC0021482) in Jones County as an approved residuals source, with an approved permitted annual dry tonnage of 15 dry tons per year (DT/yr). ➢ Addition of the Town of Bridgeton WWTP (Permit No. NC0074837) in Craven County as an approved residuals source, with an approved permitted annual dry tonnage of 5 DT/yr. ➢ Reorganization of fields in the permit with subsequent acreage changes, including the combination of aspects of previously permitted Fields 3, 4 and 5 into Field 345. ➢ Addition of Field 10 in Duplin County, adding an additional 12.8 acres to the program. The modified Attachments A and B shall be effective from the date of certification until June 30, 2027 (i.e., expiration date of the current permit), shall void Attachments A and B of Permit No. WQ0030998 issued May 3, 2021, and shall be subject to the conditions and limitations as specified therein, as well as those specified in the above -referenced permit. Please replace Attachments A and B of Permit No. WQ0030998 that was issued May 3, 2021 with the modified Attachments A and B. Please note that the Permittee shall submit a renewal application no later than January 1, 2027. Please note that only the maps for new land application sites are attached to this letter. Smith April 18, 2022 Page 2 of 2 If any parts, requirements or limitations contained in this certification are unacceptable, the Permittee has the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request shall be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, NC 27699-6714. Otherwise, this certification shall be final and binding.
