Attached is the document that I submitted to the Fayetteville Regional Office with all DMRs signed and dated. Outfall IDs
and electronic signatures could not be entered on the uploaded form to the SW website. Do you still need a revised set
of DMRs for the Gardner Quarry V 6-month period-2021?

Bethany,
I'm finally back in my office and am getting ready to upload and submit a revised 3rd quarter-2021 DMR for the Senter
Sand & Gravel Mine, COC# NCG020156. However, for the below message referencing the Gardner Quarry 3rd or 4t"
quarter-2021, my scanned copies show that both DMRs submitted contained original signatures. Do you have a pdf of
the DMR in question? I'd like to get these DMRs straight, and I'm sorry for the carelessness of the original Senter
submittal.
