Dear Ms. Groome:
Division personnel have reviewed and approved your application for minor modification of the subject
permit. Accordingly we are forwarding the attached modified permit page. Please remove the existing
"Effluent Limitations and Monitoring Requirements" page and replace it with the one attached to this
letter.
This modification reduces the effluent monitoring frequencies for Biochemical Oxygen Demand
(BOD5), Total Suspended Solids (TSS), Ammonia Nitrogen (NH3-N) and Fecal Coliform from daily
(5/week) to 2/week monitoring. The modification was based upon an evaluation of three years of
facility effluent monitoring data. The facility's performance satisfies the criteria established in the
"DWQ Guidance Regarding the Reduction of Monitoring Frequencies in NPDES Permits for
Exceptionally Performing Facilities" as approved by the Director of the Division of Water Quality on
October 22, 2012, and justifies reduced monitoring for these parameters. The modified monitoring
frequencies will become effective on June 1, 2013.
This modification is issued pursuant to the requirements of North Carolina General Statute 143-215.1
and the Memorandum of Agreement between North Carolina and the U.S. Environmental Protection
Agency dated October 15, 2007 (or as subsequently amended).
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or permits
required by the Division of Land Resources, the Coastal Area Management Act or any other Federal or
Local governmental permit that may be required.

Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to
the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between North Carolina and the U.S. Environmental Protection Agency dated October 15, 2007 (or as
subsequently amended).
The following changes have been incorporated into this permit renewal:
• The Jordan Lake Nutrient Management Strategy rules adopted in Aug. 2009 prompted a stream
reclassification for North Buffalo Creek from C; NSW to WS-V; NSW. See the Supplement to
Permit Cover Sheet.
• Cr, Ni, Se, Fe and chloride sample results showed no reasonable potential to violate Water Quality
Standards (WQS) and were removed from the permit.
• Mercury sampling results showed reasonable potential to violate WQS. Mercury monitoring remains
in the permit with a daily maximum limit of 36 ng/L and a monthly average limit of 12 ng/L.
Chronic and acute limits were required by US EPA Region N. Monitoring was reduced to quarterly.
• Cu and Zn sampling results showed reasonable potential to violate WQS, however, they are action
level parameters and limits are set in conjunction with toxicity test results. North Buffalo WWTP is
passing its toxicity tests so limits are not necessary. Both parameters remain in the permit and
sampling was reduced from monthly to quarterly.
• All CN samples reported by the City were less than 20 ug/L, therefore, the limits for CN were
removed and monitoring was reduced to quarterly in accordance with Division guidance. The City
should consider using improved equipment or sampling techniques to achieve a lower PQL.

• Fluoride did not show reasonable potential to violate WQS but predicted concentrations were greater
than 50% of the allowable concentration. Since this is not a required Pretreatment Long Term
Monitoring Program parameter it will remain in the permit with a reduced sampling frequency of
quarterly and no limitation.
• A second footnote has been added to Section A. (2.) Combined Limitations for Nutrients— Outfall
C01 to recognize Session Law 2011-394, H.B. 119.
• Section A. (4.) on the Effluent Pollutant Scan now designates the three years in which the scans are to
be performed. Please review.
• Section A. (7.) (d.) on Combined Annual Delivered Loads (Ib/yr) for Total Phosphorus and Total
Nitrogen requests that the Combined Delivered Loads be labeled in the DMR report.
• Section A. (8.) was added to address the City of Greensboro's Nitrogen Optimization Plan.
Please note that the receiving stream is listed as an impaired waterbody on the North Carolina 303(d)
Impaired Waters List. Addressing impaired waters is a high priority with the Division, and instream
data will continue to be evaluated. If there is noncompliance with permitted effluent limits and stream
impairment can be attributed to your facility, then mitigative measures may be required.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable
to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days
following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative
Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is
made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or permits

City of Greensboro North Buffalo [NC0024325] Reduced Monitoring Request
The City of Greensboro hereby requests designation as an "Exceptional Performing Facility"
and reduced monitoring for the following parameters listed as daily monitoring in the current
North Buffalo NPDES permit: CBOD5, TSS, NH3-N, and fecal coliform. A spreadsheet
summarizing the applicable data is also part of this submittal.

Frequencies in NPDES Permits for Exceptionally Performing Facilities]
individual NPDES facilities shall submit a written request to modify their NPDES permit
specifying which parameters are to be considered for reduced monitoring . Permit holders
must include with their requests sufficient data, statistical analyses, and other information to
support the justification for reduced monitoring. NPDES permit staff shall review the application
and supporting information. Approval of reduced monitoring frequency shall be granted if all the
criteria below are met:
• The facility has no more than one civil penalty assessment for permit limit violations for each
target parameter during the previous three years.
o During CY 2010 to 2012, the North Buffalo POTW was not assessed a civil
penalty for any of the four target parameters as there were no violations.
• Neither the permittee nor any of its employees have been convicted of criminal violations of
the Clean Water Act within the previous five years.
o No City of Greensboro employees have been convicted of criminal
violations of the Clean Water Act within the previous five calendar years
(2008-2012)
• The facility is not currently under an SOC for target parameter effluent limit noncompliance.
o The North Buffalo POTW Is not currently under an SOC for CBOD5, TSS,
NH3-N, or fecal coliform violations.
• The facility is not on EPA's Quarterly Noncompliance Report for target parameter limit
violations.
o The City of Greensboro has not been notified that the North Buffalo POTW
is on EPA's Quarterly Noncompliance Report for CBODB, NH3-N, TSS or
fecal coliform and there were no violations In CY2010 2012.
• For BOD5, CBOD5, TSS, NH3-N and TSS, the three year arithmetic mean of effluent data
must be less than fifty percent of the monthly average permit limit. For fecal coliform or
enterococci, the three year geometric mean must be less than 50 percent of the monthly
average permit limit. For parameters with summer and winter limits, an annual arithmetic
mean of the seasonal limits may be used in the calculation.
o The North Buffalo CY2010-2012 effluent concentrations are less than 50%
of the NPDES monthly average limits as follows [see attached data
