Division personnel have reviewed and approved your application for the subject permit. Accordingly, we are
forwarding the attached NPDES discharge permit. This permit is issued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S.
Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).

If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to
you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of
this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North
Carolina General Statutes, and filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh,
North Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may require
modification or revocation and reissuance of the permit. This permit does not affect the legal requirements to obtain
other permits which may be required by the Division of Water Quality or permits required by the Division of Land
Resources, the Coastal Area Management Act or any other Federal or Local governmental permit that may be

SUPPLEMENT TO PERMIT COVER SHEET
Transylvania Utilities, Inc., is hereby authorized to:
1. Continue to operate an existing 0.3 MGD wastewater treatment system with the
following components:
• Concrete flow splitter
• Dual aeration basins with mechanical aerators
• Dual rectangular clarifiers
• Chlorine disinfection equipment
• Flow meter
• Sludge digester with holding & drying beds
This system discharges from outfall 001 at the Transylvania Utilities WWTP on
U.S. Highway 276 South near Connestee Falls in Transylvania County.
2. Continue to operate an existing 0.02 MGD wastewater treatment system with the
following components:
• Extended aeration package plant with tertiary filter
• Chlorine disinfection equipment
• Aerated sludge holding
This system discharges from outfall 002 at the Transylvania Utilities WWTP on
U.S. Highway 276 South near Connestee Falls in Transylvania County.
3. Discharge from said treatment works at the location specified on the attached
map into the French Broad Rii r (outfall 001) and Lower Creek (outfall 002).
Both streams are classified e-Trout waters in the French Broad River Basin.

A. (3.) DISINFECTION CONDITION
In the event that violations of the water quality standard for fecal coliform occur as a result
of this discharge, disinfection will be required immediately. This permit will be amended
as necessary to set effluent limits for fecal coliform or to protect water quality in the
receiving stream.

WWTP class: II
WWTP type: Outfall 001: located at 5999 Greenville Highway near
Connestee Falls is a class II facility and rated as a 0.300
MGD extended aeration activated sludge system with: bar
screen; concrete influent flow splitter, dual aeration basins
with floating mechanical aerators; dual rectangular clarifiers
with sludge returns; aerobic digester; chemical feed for
sodium hypochlorite (12% liquid bleach) disinfection;
chlorine contact chamber; chemical feed for liquid sodium
bisulfite dechlorination; Isco Ultrasonic 3010 flow meter;
refrigerated composite sampler; (the existing sludge drying
beds are not being utilized — plans are in place for use of a
sludge box to dewater excess sludge).
Outfall 002: located off of Walnut Hollow Road near
Connestee Falls is a Clow Aer-O-Flo, classified as a class II
facility and rated as a 0.020 MGD extended aeration package
plant with: influent bar screen, influent Isco Ultrasonic 3010
flow meter; aeration basin with dual blowers providing
diffused air; clarifier with 2 airlift sludge returns; aerobic
digestor; upflow mixed media tertiary filter (16 sq ft) with
backwash blower; tablet chlorinator; chlorine contact
chamber; and tablet dechlorinator. The discharge point is
1740 feet south of the WWTP along Walnut Hollow Road.
Collection System: WQCS00219
The collection system consists of a total of 34.46 miles:
28.12 miles of gravity sewer and 6.33 miles of force main,
with 17 duplex pump stations with telemetry. The as built
flow is 0.0325 MGD.
Ouffall 002 Expansion: Division received this request on 3-26-2010.
The system currently has 64 connections and received only
domestic wastewater. The rated capacity of the existing
plant is 20,000 gpd. The current flows average 9,000 to
10,000 gpd. Transylvania Utilities, Inc. is requesting to
expand the permit in order to provide service to 118
additional residential parcels. The infrastructure for these
additional connections is currently installed and
commitments exist to the actual lot owners. The additional
connections translate to a 43,200 gpd increase to the
WWTP's rated capacity (based on a unit flow allocation of
360 gpd/connection).
Emergency generators:
Located next to Ouffall 001 WWTP is a 80 KW Generac LP
gas emergency generator that backs -up the controls of
Ouffall 001 WWTP's main components and lift stations. This
generator self -tests weekly. There are also four portable
generators: one 85 KW diesel, two 30 KW gas and one 45
KW gas.
