However, additional information is required before the review may be completed. Please address the item on the attached page no later than the close of business on May 23, 2022. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant’s responsibility. In addition, any omissions made in responding to the outstanding item in Section A, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete pursuant to 15A NCAC 02T .0107(e)(2). Please reference the subject application number when providing the requested information. During communications with Mr. Don O’Mara regarding the now-resolved air mixing question, it was determined that the ultraviolet (UV) disinfection system was modified from the permitted two 62 gallon per minute (GPM) disinfection units to three 40 GPM UV disinfection units. This change is a minor modification, and the Non-Discharge Branch does not have any records of this modification being submitted for review. Please provide information regarding the UV disinfection system including equipment type, treatment configuration, calculations, operation and maintenance requirements. Please also provide information for any additional or modified piping or appurtenances that were required for installation of the modified system. Please submit application form HRIS 06-16, signed and sealed engineering documents pursuant to 15A NCAC 02T .0704. Our understanding is that this equipment has already been installed, as such please also provide the date it was installed, an executed engineering certification, and the updated Operation and Maintenance (O&M) plan.
