Dear Ms. Raup-Plummer:
We offer the following responses and information in reply to your letter of November
29, 2021, to Mr. Grady Fulcher:
B. Application:
1. It is my understanding that the ORC for this system, Mr. Don O’Mara has contacted
the Operator Certification section and the operator designations have been corrected
and updated.
2. The previous Site Map was of a poor quality, which made it difficult to determine
where the monitoring wells were located in relation to the compliance and review
boundaries. To correct this situation, the Genesis HOA retained a registered land
surveyor to accurately locate the wells, infiltration areas, and property lines. Section
VIII of the application has been revised to reflect information obtained by the survey,
and a copy of the revised page is enclosed. The listed latitude and longitudes were also
revised slightly, based on the surveyed locations of the wells.
MW-1 is located 1.40 feet from the Review Boundary, and is assumed to be on the
Review Boundary given the boundary of the infiltration site is not a precise line.
MW-2 is located 3.65 feet from the Review Boundary, and is now noted in the revised
Section VIII as being inside the Review Boundary.
MW-3 is located on the Compliance Boundary, as originally noted in Section VIII.

D. Additional Documentation:
It is my understanding from Mr. Don O’Mara, the ORC for the facility, that he contacted
you about the aeration system and the issue/question has been resolved.
Thank you for your patience regarding this response. As Mr. Fulcher explained in a
previous email, it has been difficult to find a surveyor that would agree to provide this
survey in a timely manner.
Please let me know if you require anything else to process this permit renewal.
