The Clean Water Solutions Permit had an expiration date of 3-16-22. In 2021, the SIU
changed names to Hazmat Emergency Response & Remediation and asked for a
modification. Pretreatment Coordinator submitted forms to change name and issue a
new permit, but apparently did not receive response from CO. A permit was issued
with new name and extended expiration date of 6-1-23 without CO approval. WIRO
recommended forms to modify permit be submitted again for CO approval. HWA is still
in review.
