Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to
the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between North Carolina and the U.S. Environmental Protection Agency dated May 9, 1994 (or as
subsequently amended).
This final permit includes the following changes from the draft permit sent to you on
June 7, 2006.
> Based on adverse comments from EPA, the Division was required to implement daily maximum NH3 as
N (ammonia) limits for non -municipal facilities, as required by the Code of Federal Regulations. This
implementation policy was adopted October 15, 2002. The daily maximum value is five times the
monthly average value (this multiplier was established based on a review of treatment capability at
various non -municipal plants). However, daily maximum values are capped at 35.0 mg/L, regardless of
the monthly average value.
> The ammonia nitrogen monthly average limits for each permitted flow have been corrected to 16.0 mg/L
[at 0.16 MGD] and 13.0 mg/L [at 0.2 MGD]. The draft mistakenly placed the daily maximum limit of
35.0 mg/L in place of the monthly average limits. The monthly average limits are unchanged from your
previous permit.
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative
Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is made,
this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division
may require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or permits
required by the Division of Land Resources, the Coastal Area Management Act or any other Federal or
Local governmental permit that may be required. If you have any questions concerning this permit,

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby
revoked. As of this permit issuance, any previously issued permit bearing this number is no longer
effective. Therefore, the exclusive authority to operate and discharge from this facility arises under the
permit conditions, requirements, terms, and provisions included herein.
The Quarterstone Farms Homeowner's Association is hereby authorized to:
1. Continue to operate an existing 0.16 MGD wastewater treatment facility with the
following components:
• Two duplex pump stations
• Bar screen
• Dual 0.08 MGD contact stabilization treatment units (each with a reaeration
tank and a secondary clarifier)
• Liquid chlorination with chlorine contact chamber
• Two blowers
• Flow recording equipment with totalizer,
This facility is located northeast of Greensboro at the Quarterstone Farm WWTP
off Huffine Mill Road near NCSR .2714 in Guilford County.
2. After receiving an Authorization to Construct from the Division, construct and
operate a 0.2 MGD wastewater treatment system.
• If the facility is to be expanded in phases, plans and specifications for the next
phase of construction shall be submitted to the Division when the flow to the
existing treatment system reaches 80% (0.128 MGD) of the installed capacity
(0.16 MGD)
3. Discharge from said treatment works at the location specified on the attached map
into Buffalo Creek, classified C-NSW waters in the Cape Fear River Basin.

A. (3) PERMIT RE -OPENER: NUTRIENT CONTROLS
The Division may, upon written notification to the Permittee, re -open this Permit in
order to incorporate or modify effluent limitations, monitoring and reporting
requirements, and other permit conditions when it deems such action is necessary to
implement nutrient management requirements such as those established through
rulemaking or USEPA approval of TMDL(s) .
A. (4) CALCULATION OF MASS LOADS
a. The Permittee shall calculate monthly and annual TN (or TP) Loads as follows:
i. Monthly TN (or TP) Load (pounds/month) = TN (or TP) x TMF x 8.34
where:
TN (or TP) = the average Total Nitrogen (or Total Phosphorus) concentration
(mg/L) of the composite samples collected during the month
TMF = the Total Monthly Flow of wastewater discharged during the month
(MG/month)
8.34 = conversion factor, from (mg/L x MG) to pounds
ii. Annual TN (or TP) Load (pounds/year) = Sum of 12 Monthly TN (or TP) Loads
for calendar year
b. The Permittee shall report monthly Total Nitrogen and Total Phosphorus results (mg/L
and pounds/month) in the appropriate discharge monitoring report for each month and
shall report each year's results (pounds/year) with the December. report for that year.

I've received and reviewed the subject draft. My only comment would be to make sure that the NH3
limits are accurate. The new monthly average limit of 35 mg/1 is more than double the previous limit @
0.16 MGD and almost triple the previous limit @ 0.2MGD.

NPDES WASTEWATER PERMIT
On the basis of thorough staff review and appli-
cation of NC General Statute 143.21, Public law
92-500 and other lawful standards and regula-
tions, the North Carolina Environmental Man-
agement Commission proposes to issue a Na-
tional Pollutant Discharge Elimination System
(NPDES) wastewater discharge permit to the
person(s) listed below effective 45 days from
the publish date of this notice
Written comments regarding the proposed per-
mit will be accepted until 30 days after the
publish date of this notice. All comments re-
ceived prior to that date are considered in the
final determinations regarding the proposed
permit. The Director of the NC Division of Wa-
ter Quality may decide to hold a public meet-
ing for the proopposed permit should the Division
receive a significant degree of public interest.
Copies of the draft permit and other supporting
information on file used to determine condi-
tions present in the draft permit are available
upon request and payment of the costs of re-

Quarterstone Farms WWTP in Guilford County.
This permitted facility discharges treated

tions in this portion of the Cape Fear River Ba-
sin.
NPDES Permit Number NC0000795, IGnder Mor-
gan Southeast Terminals LLC, Greensboro Ter-
minal 1 has applied fora permit renewal for a
facility locatedinGuilford County discharging
treated wastewater into an Unnamed Tributary
of the East Fork Deep River in the Cape Fear
River Basin. Currently, no parameters are water
.quality limited., however this discharge may af-
fect future allocations in this portion of the re-
ceiving stream.
NPDES Permit Number NC0022209, Motiva En-
terprise LLC, Greensboro Terminal has applied
for a permit renewal for a facility located in
Guilford County discharging treated wastewa-
ter into an Unnamed Tributary of Long Branch
Creek in the Cape Fear River Basin. Currently, no
parameters are water quality limited, however
this discharge may affect future allocations in
this portion of the receiving stream.
The City of High Point (P.O. Box 230, High Point,
NC 27261) has applied for renewal of NPDES
permit number NC0081256 for the Frank L.
Ward WTP in Guilford County. This permitted
facility discharges treated wastewater to in the
Richland Creek in the Cape Fear River Basin.
Some parameters may be water quality limited,
which may affect future allocations in this por-
tion of the Cape Fear River Basin.

Sludge from the Quarterstone Farms wastewater treatment plant are disposed of in the following manner:
Solids are collected in the sludge holding tank and digested aerobically. The excess solids are
periodically pumped and hauled by a licensed septic pumper contractor and disposed of at the city of
Greensboro wastewater treatment plant.

representation, or certification in any application, record, report, plan, or other document files or required to be
maintained under Article 21 or regulations of the Environmental Management Commission implementing that
Article, or who falsifies, tampers with, or knowly renders inaccurate any recording or monitoring device or method
required to be operated or maintained under Article 21 or regulations of the Environmental Management
Commission implementing that Article, shall be guilty of a misdemeanor punishable by a fine not to exceed

Dear Permittee:
The subject permit's expiration date is June 30, 2006. Federal [40 CFR 122] and state (15A NCAC
2H.0105(e)) regulations require that an application for permit renewal be filed at least 180 days prior to expiration
of the current permit. To satisfy this requirement, your renewal package should have been sent to the Division
postmarked no later than January 1, 2006.
As of this date, the Division has not received your renewal application. This is a violation of Part II. B. 10.
of your permit, which states "Any permittee that has not requested renewal at least 180 days prior to
expiration...will subject the permittee to enforcement procedures as provided in NCGS 143-215.6 and 33 USC
1251 et. seq.". To prevent an assessment of civil penalties you must submit a completed permit application
(see enclosed forms) no later than February 28, 2006.
Use the enclosed checklist to complete your renewal package. The checklist identifies the items you must
submit with the permit renewal application. If all wastewater discharge from your facility has ceased and you
wish to rescind this permit [or if you have any questions] please contact Charles H. Weaver, Jr. of my staff. His
telephone number, fax number and e-mail address are listed at the bottom of this page.
