Describe the existing conditions on the site and the general land use in the vicinity of the project at the time of this application: * The unnamed tributary to Greenfield Lake passes through a double 60" corrugated metal pipe culvert under Medical Center Drive. At the location where maintenance is to be performed, adjacent to the stream's west side is a gas station with convenience store and a Bojangles restaurant. On the stream's east side is a Medical Office and an undeveloped, wooded lot. 4b. Explain the purpose of the proposed project:* The proposed project is to perform maintenance on an unnamed tributary to Greenfield Lake. Maintenance to be performed includes removal of sediment and vegetation within the stream channel to restore the channel to original cross section. Work will be performed within existing drainage easements owned by the City if Wilmington. 4i. Describe the overall project in detail, including indirect impacts and the type of equipment to be used: * The total length of stream to be impacted is 200 linear feet. 125 feet is on the north side of Medical Center Drive. 75 feet is on the south side of Medical Center Drive. There are 2 — 60" corrugated metal pipe culverts that run under Medical Center Drive and connect the two segments of stream. The average width of the channel to be impacted is approximately 20 feet for a total area of disturbance of approximately 4,000 Square Feet. The depth of sediment over the project area to be removed is on average 1.5 feet. It is estimated that the volume of sediment to be removed is approximately 240 CY. Work will be completed via excavator from the top of bank, and hand tools. The removed sediment and debris will be disposed of at the City of Wilmington's operations & maintenance facility located on River Road. 5. Jurisdictional Determinations 5a.
