KED PROJECT #21-022 APRIL 2022 KENNERLY ENGINEERING & DESIGN, INC. April 1, 2022 Jenny Graznak Assistant Regional Supervisor NC Division of Water Resources Department of Environmental Quality 450 Hanes Mill Road Winston-Salem, NC 27103 RE: Project Name: Ridgemont Subdivision Section 3 & 4 County: Davie City: Mocksville Address: 604 Whitney Road Submitted by: Kennerly Engineering & Design, Inc. Ms. Graznak, Kennerly Engineering & Design, Inc. (KED) submits the following application for your review, comments and/or approval. Please find attached two (2) copies of the application and calculations for the above referenced project. KED has been retained as the Design Engineer and Engineer of Record for the Ridgemont Subdivision Section 3 & 4 located in Mocksville, NC. The project is a continuation of an existing subdivision that began in the 1970's. Section 3 & 4 are the last sections of the development that have not been constructed. Section 3 & 4 will consist of single-family homes. This project will include approximately 4,062 LF of 8" diameter sanitary sewer line. The proposed sanitary sewer lines will be installed within the right-of-way of the road system except for the outfall connections to the existing sanitary sewer system owned by the Town of Mocksville. The existing sanitary sewer outfalls are located within the 100 YR floodplain of Elisha Creek and an. unnamed tributary of Elisha Creek. The sanitary sewer for this project will be public and will be owned and maintained by the Town of Mocksville. There will be three (3) sanitary Sewer Outfalls for this section of the subdivision. The outfalls will include stream crossings, wetlands and floodplain disturbance. KED has contracted an Environmental Group, NW Geoscience, PC, to permit the stream crossings, wetlands and floodplain disturbances. NW Geoscience has met on site with the Army Corp of Engineers and delineated the areas of the crossings and made he determinations with Corp. NW Geoscience has filled for the NW 58 Permits for the sanitary sewer outfalls and will contact the NC Division of Water Resources to obtain the 401/404 certifications that will also be needed once the Corp permits are obtained. An Erosion Control Permit was issued by NCDEQ on September 20, 2021 for the land disturbance activities for the project. Public Water Supply (PWS) has also issued a permit for Phase 3A of the project and the remaining portion of the subdivision is under review by PWS currently. Should you have any questions, please do not hesitate to contact us at 336.775.2118. Sincerely, ‘0,1--Cri—dor Timothy S.
