During inspection it was found that the lagoon was
overflowing. Water was entering the adjacent storm
drain.
Chlorine Levels in Lagoon checked 65ug/L above limit
Chloring levels in storm water retention pond 44ug/L
Ori Tuvia left facility stated need to find a way to stop
overflow
Opened Recirculation valve on lagoon pond which sends
water to the pre -sedimentation basin. This stopped the
overflow of the Lagoon.
January 26th,2022 Ori Tuvia sent a zoom meeting request for January
26th,2022 2:00 pm — 3: 00 pm
Ori Tuvia resend a request for zoom meeting for
January 27th,2022 11:00 am — 12:00 pm due to NCDENR
partners scheduling conflicts.
January 26th,2022 Lagoon Chlorine level checked 43ug/L
Discharging Normally... Level reduced to maximum level
of free board.
January 27t1,2022 Lagoon Level had increased
Discharging Continued
It was found that the Stormwater from the chemical
containment area is entering the lagoon. We have a
stormwater system connected to discharge system
ahead of Lagoon.
ZOOM Meeting Ori Tuvia, Andrew Pitner, Corey
Basinger, Raymond Whitner Talked about corrections
and responses.

Ray Whitner & Clinton Cook onsite to perform testing
on filter flow. Also, to talk about reporting the recycled
flow on eMOR. Findings all Normal
Town crew removed solids from filters to take to the
landfill.
Asked about putting solids in containment area used
when pre -sedimentation basin was dredged of material.
Was informed that couldn't be done because of the
possibility of water from containment entering the
storm drain system.
Letter of actions and correction measures. To
send to state
Zach keys onsite to give possible options to remove
solids. Asked to check on status of WTP Land
Application permit.
Found that Land Application permit was expired.
Looking at option to use BIO tubes to hold solids and
dewater for solids removal. He is also working to add
the WTP to his Land Application permit. To enable
them to apply solids.

Asked why we haven't responded to NOV Letter.
Replied I never got a NOV letter. Maria Schutte Asked
about landfill records. I requested that Ori send me a
digital copy of letter. I said I would take letter and work
toward finding the letter.
Received email from Ori Tuvia with letter attached
dated 2-10-22.
Went to utility directors' office to get landfill tickets and
ask if there had been a NOV letter delivered
to his office. He had not received any letter.
Went to Town Managers office to check to see if it had
been received in that office. It had not been received
there.
I requested a copy of the certified letter signature card
from Ori Tuvia.

The Signature card showed it was signed by a USPS
Agent for delivery. No one from the town had signed
that they received it.
Started Compiling information for response to letter.
