On March 22, 2022, the Division of Energy, Mineral, and Land Resources (DEMLR) conducted
a site inspection at the subject facility. As a result, Division staff have determined that an
individual industrial stormwater permit is required. A DEMLR file review confirmed that an
application was received by DEQ on November 18, 2004. As this application is out of date,
please submit new application forms such that current site operations are up to date and properly
reflect current site conditions.

Inspection Summary:
An inspection was conducted to determine if a permit is required at the facility. It was determined a permit is required and an
application package should be submitted as soon as possible.
The facility has two retention ponds on site, one on the West side of the property that catches most of the facilities runoff
and flows to the pond in channels that flow through the mulch piles. The pond has been filled completely with mulch and
sediment and has small trees growing in it. The outlet from that pond runs into a tributary (this a sample point where it runs
into the trib) that has a distinct color difference from the mulch runoff. This runoff dumps into a neighbors pond just over the
property line to the South. The second retention pond found at the South end of the property below an equipement area that
stores old equipement,oil and other materials that are all stored in the open. Runoff runs through that area and a small part
of the mulch pile area into that small pond (this is a sample point). This equipement area should be cleaned up and all waste
and new oil should be stored inside or under cover with any containers stored outside cleaned up immediately. There is a
fairly sinificant part of the mulch storage area that runs to the East of the property down a slope and into Lynch Creek (A
sample point should be designated where runoff runs to the creek)
