Subject: Permit Contact Update Request for NCG201488
Unfortunately we cannot process the Permit Contact Update Request for permit no. NCG201488 submitted
on 4/5/2022 any further. A Stormwater Program staff member will be in touch with more information.
Comments during review: This is the wrong form to request updates to an NCGO1 Construction Permit COC
(this is for an industrial permit). Please use the NCGO1 Modification Form

(https://edocs.deq.nc.gov/Forms/NCGOI-Modification) and upload a Legally Responsible Owner Change form
(there is a link on that form to that document). In the 'Additional Explanation' box, just let us know that David
Vannoy will also be the new Site Contact. The correct permit number is 'NCC20148 8', not 'NCG201488' (prefix
is always 'NCC' for construction permits).
