Dear Mr. Campbell:
Division staff have reviewed and approved your application for an NPDES discharge permit. Accordingly,
the Division is forwarding the subject NPDES permit. This permit is issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and
the U.S. Environmental Protection Agency dated May 9,1994 (or as subsequently amended).
The following items appear in this FINAL permit:
• Chronic toxicity test language has been updated, requiring different follow-up procedures after a test
failure.
• The instream monitoring requirements continue to be waived as long as the permittee maintains
membership in the Middle Cape Fear River Basin Association. In addition, instream monitoring has
been deleted for fecal coliform and conductivity, based on the updated NPDES Permitting Strategy
for 100% domestic facilities.
• The permit expiration date has been changed to 9/30/06.
• Although not listed in the effluent sheet of the draft permit, this final permit continues the
requirement for influent monitoring for BOD and TSS in order to assess the 85% removal
requirement.
• Based on EPA comments on several draft permits, this final permit reflects a new Special Condition
(A (3) - Ammonia Reopener) which will add a weekly average limit in addition to the monthly
average limit for ammonia. This new limit will be added during this permit term (if reasonable
potential exists), or at the next permit renewal.
Please note that mercury continues to be a water quality concern throughout North Carolina. NPDES
permittees have worked with the state to reduce potential risks from this pollutant, including tasks associated
with collecting and reporting more accurate data. The most commonly used laboratory analysis (EPA
Method 245.1) has a detection limit of 0.2 µg/L while the current water quality standard is an order of
magnitude lower at 0.012 µg/L. A more recendy approved analytical test (EPA Method 1631) should
produce a detection limit below the level of the standard. This will allow the Division to assess potential
water quality impacts from discharges more accurately. Therefore, beginning on or before September 1,
2003, you will be required to begin using EPA Method 1631 when analyzing for mercury according to terms
of your NPDES permit.

If any parts, measurement frequencies, or sampling requirements contained in this permit are unacceptable to
you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following
receipt of this letter. This request must be in the form of a written petition, conforming to Chapter 150B of
the North Carolina General Statutes, and filed with the office of Administrative Hearings, 6714 Mail Service
Center, Raleigh, North Carolina 27699-6714. Unless such a demand is made, this permit shall be final and
binding.
Please take notice that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. The Division may require modification or

permits which may be required by the Division of Water Quality, the Division of Land Resources, the Coastal
Area Management Act, or any other federal or local governmental permit.

NOTES:
1 • Upstream = at NC Highway 217. Downstream = approximately 1400 feet below discharge.
2. The monthly average effluent BOD and Total Suspended Solids concentrations shall not exceed 15% of the
respective influent value (85% removal).
3. A weekly average ammonia limit will be developed at a later date; refer to Special Condition A. (3).
4. The daily average dissolved oxygen concentration as measured in the effluent shall not be less than 5.0 mg/L.
5. Chronic Toxicity (Ceriodaphnia) limit at 1% with testing in March, June, September, and December (see Special
Condition A. (2)).
6. The pH shall not be less than 6.0 standard units nor greater than 9.0 standard units.
7. Instream monitoring shall be waived as long as the permittee maintains membership in the Middle Cape Fear
River Basin Association (MCFRBA), which conducts comprehensive instream monitoring. If the permittee
cancels membership in the MCFRBA, the facility shall begin collecting the listed instream samples immediately
at a frequency of 3/week Oune-September) and weekly (October -May).
There shall be no discharge of floating solids or visible foam in other than trace amounts.

A. (2) CHRONIC TOXICITY PERMIT LIMIT (Quarterly) — OUTFALL 001
The effluent discharge shall at no time exhibit observable inhibition of reproduction or significant mortality to
Ceriodaphnia dubia at an effluent concentration of 1.0%.
The permit holder shall perform at a minimum, quarterly monitoring using test procedures outlined in the "North
Carolina Ceriodaphnia Chronic Effluent Bioassay Procedure," Revised February 1998, or subsequent versions or "North
Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure" (Revised -February 1998) or subsequent versions.
The tests will be performed during the months of March, June, September and December. Effluent sampling for
this testing shall be performed at the NPDES permitted final effluent discharge below all treatment processes.
If the test procedure performed as the first test of any single quarter results in a failure or ChV below the permit limit,
then multiple -concentration testing shall be performed at a minimum, in each of the two following months as
described in "North Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure" (Revised -February 1998) or
subsequent versions.
The chronic value for multiple concentration tests will be determined using the geometric mean of the highest
concentration having no detectable impairment of reproduction or survival and the lowest concentration that does
have a detectable impairment of reproduction or survival. The definition of "detectable impairment," collection
methods, exposure regimes, and further statistical methods are specified in the "North Carolina Phase II Chronic
Whole Effluent Toxicity Test Procedure" (Revised -February 1998) or subsequent versions.
All toxicity testing results required as part of this permit condition will be entered on the Effluent Discharge
Monitoring Form (MR-1) for the months in which tests were performed, using the parameter code TGP3B for the
pass/fail results and THP3B for the Chronic Value. Additionally, DWQ Form AT-3 (original) is to be sent to the
following address:

Completed Aquatic Toxicity Test Forms shall be filed with the Environmental Sciences Branch no later than 30 days
after the end of the reporting period for which the report is made.
Test data shall be complete, accurate, include all supporting chemical/physical measurements and all
concentration/response data, and be certified by laboratory supervisor and ORC or approved designate signature.
Total residual chlorine of the effluent toxicity sample must be measured and reported if chlorine is employed for
disinfection of the waste stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring is required, the
permittee will complete the information located at the top of the aquatic toxicity (AT) test form indicating the facility
name, permit number, pipe number, county, and the month/year of the report with the notation of "No Flow" in the
comment area of the form. The report shall be submitted to the Environmental Sciences Branch at the address cited
above.
Should the permittee fail to monitor during a month in which toxicity monitoring is required, monitoring will be
required during the following month.
Should any test data from this monitoring requirement or tests performed by the North Carolina Division of Water
Quality indicate potential impacts to the receiving stream, this permit may be re -opened and modified to include
alternate monitoring requirements or limits.
NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum control organism
survival, minimum control organism reproduction, and appropriate environmental controls, shall constitute an invalid
test and will require immediate follow-up testing to be completed no later than the last day of the month following the
month of the initial monitoring.

A. (3) . AMMONIA REOPENER
The Division may re -open this permit to require weekly average limits for ammonia. After calculating allowable
concentrations, the Division will perform an analysis of past ammonia data to determine if there is a reasonable
potential 'for this discharge to exceed these potential limits. If there is, this permit will be re -opened. If there is not,
the permit will not be re -opened, but will contain weekly average limits for ammonia upon renewal.

■ The City of Dunn is requesting a major permit renewal foYa 3.75 MGD extended aeration
municipal wastewater treatment facility serving a population of 9,931. Per the renewal
application, there have been no changes in the operation of the treatment plant since the
permit was last renewed, and no planned upgrades or expansions are anticipated during
the next 5 years. The facility discharge has been averaging 2.1 MGD, which represents
56% of design flow. The current discharge is primarily treated domestic wastewater, with
daily industrial flow < 0.05 MGD (< 2.4% of actual flow). There is no pretreatment
program requirement at this time. Residuals are land applied under permit WQ0006101.

■ This facility discharges to the Cape Fear River. Although the city is located within
subbasin 030618, the discharge is piped to the Cape Fear River in subbasin 030613. The
receiving stream segment is classified as water supply (WS-V), with the City of Fayetteville
water intake located approximately 20 miles downstream. The receiving segment is not
listed as impaired on the state's 303(d) listing. However, there are some water quality
concerns in the middle segment of the Cape Fear River. A field calibrated QUAL2E model
developed by DWQ during the first basinwide planning cycle indicated that assimilative
capacity for oxygen -consuming wastes had been reached in the segment of the Cape Fear
River from Erwin to Lock and Dam #3. It was recommended that any expanding
municipal discharge >1 MGD be given limits of BOD5= 5 mg/1 and NH3-N = 1 mg/1. The
QUAL2E model is currently being reviewed to determine if improvements in the calibration
can be made.
Permit Development
• Historical. A permit modification issued on 8/18/00 extended the permit expiration date
to 9/30/01 based on basin alignment. Streamflow is regulated at Jordon Lake Dam at a
s7Q10 flow= 586.6 cfs. At a WWTP design flow of 3.75 MGD, the discharge represents
only a 1% instream waste concentration during summer low flow conditions. Quarterly
metals monitoring was dropped in the 1996 permit renewal based on wastewater
composition (primarily domestic with no SIUs), lack of previous metals detection, large
instream dilution (IWC= 1%), and quarterly chronic toxicity testing requirement.
• Renewal Application. The permit renewal application (NC Standard Form A; permitted flow
> 1 MGD) was dated 3/27/01 and received on 4/2/01.
■ Instream Monitoring. The facility maintains membership in the Middle Cape Fear River
Basin Association (MCFRBA), which conducts comprehensive instream monitoring under
an MOA with the Division. Hence, instream monitoring for this facility has been waived
since 9/1/98, for as long as the facility maintains MCFRBA membership.
• Permit Limits. No changes proposed from last permit.
■ Water Quality Limited Parameters: BOD, NH3, TRC
DMR Data Review
• Effluent Chemical/Physical Data. For the calendar period 7/00-6/01, the facility was
compliant with monthly average limits. Monthly average values ranged as follows: flow
(1.6-2.g MGD); BOD (2-11 mg/1); NH3-N (<0.1-1.9 mg/1); TSS (5-20 mg/1); Fecal (1-11
org/100m1); TN (1-6 mg/1); TP (0.5-1.7 mg/1).
• Effluent Toxicity Data. Data does not indicate a chronic toxicity problem. The facility has
passed 16 of 17 quarterly chronic toxicity tests between 1998-2001, at an instream waste
concentration of 1 %.
■ Instream Data. This data is collected by the MCFRBA monitoring coalition. The current
monitoring configuration includes an upstream station at NC217 (B6840000, located
downstream of Erwin and upstream of Dunn). The closest downstream station (B7480000
at Hoffer plant intake) is located >10 miles downstream of the outfall. Instream data for
these stations collected during Summer 20000 reports the lowest instream DO at 6.4
mg/1, well above the instream standard of 5.0 mg/1.
• Compliance Data. The facility reported several pH violations for the period 7/99-6/00,
and pH control equipment (soda ash feed) was installed to correct the problem. For
calendar year 2000, the facility reported 5 overflows totaling 2.1 MG. The facility noted
that inflow and infiltration work is continuing, with $125,000 budgeted for sewer system
repairs.
Fayetteville Regional Data
• The FRO conducted a Compliance Evaluation Inspection of the facility on 5/11/01. The
facility received Satisfactory ratings for all program components, including operation and
maintenance.
• The FRO submitted a staff report dated 5/23/01. The FRO recommends permit renewal
consistent with the basin plan.

re the weekly avg for ammonia, I just heard that we have a resolution.
Apparently, you (NC) will send us a letter confirming that and we'll

The main stem of the Cape Fear River at this point is used primarily for
fish and wildlife propagation, fishing and boating. The City of
Fayetteville has a water intake approximately 20 miles downstream.
Access to this stretch of river is limited.
PART II - DESCRIPTION OF WASTES AND TREATMENT WORKS

The existing WWTP includes: three (3) influent screw pumps rated at
1.75 MGD each and one (1) suction lift pump rated at 5.25 MGD, a
mechanical bar screen 2' - 6' width channel, an aerated grit removal and
250 GPM girt washer, influent flow meter - 12" throat parshall flume
with pressure transducer, followed by a flow divider box which feeds a
coarse bubble activated sludge aeration basin with four (4) cells, three
(3) final clarifiers 60' diameter, dual chlorine contact basin - 40 minutes
detention time at 3.75 GPD, dechlorination system, effluent pumps -
three (3) vertical turbine pumps and river cascade discharge. Sludge
handling facilities include aerobic digester - 150,000 gallon capacity,
secondary aerobic digester/sludge holding tank - 500,000 gallon
capacity, a dissolved air flotation (DAF) sludge thickener, and six (6)
gravity sludge drying beds (no longer utilized).

Hi Belinda- please check out the attached Fact Sheet and let me know if
the Region is ok with it. There are only very minor changes to this
permit (e.g., update expiration date, update toxicity test language).
All limits and monitoring frequencies remain the same. In your staff
report dated 5/23/01 you recommended that we put in language requesting
a plan of action for I&I to be submitted within 90 days- please add more
specifics as to what you would expect them to submit. I have not added
this yet for a few reasons. The flow limit is a monthly average, which
they have been compliant with, and they are currently well below the
design flow (currently averaging about 56% of design flow). We have
generally requested I&I studies when a facility is requesting an
expansion, and we want to ensure that they need the additional capacity
to meet new flow and not for I&I problems. The facility has also
reported that I&I work is continuing, with $125,000 budgeted for sewer
system repairs. If possible, I'd like to notice this for next Wednesday
(March 6), so let me know what you think. Thanks.

NARRATIVE DESCRIPTION
The City of Dunn Blackriver Wastewater Treatment Facility has a permitted capacity of 3.75
MGD. The treatment plant components include: influent pumping, mechanical bar screen, aerated
grit removal, influent metering, coarse bubble activated sludge aeration, final clarification, chlorine
contact, dechlorination, effluent pumping, effluent metering, and river cascade discharge. Sludge
handling facilities include: a sludge recirculation sludge wasting pump station, a dissolved air
floatation sludge thickener, an aerobic digester, a secondary aerobic digester/sludge holding tank,
sludge drying beds, and a sludge truck loading station. Details of the major components of the
WWTP are as follows:
• Influent pumps - 3 screw pumps rated at 1.75 MGD each and one suction lift pump rated
at 5.25 MGD.
• Mechanical Bar Screen - 2'-6' width channel.
• Aerated grit removal -12' x 12' x 12' SWD chamber and 250 GPM grit washer.
• Influent metering -12" throat parshall flume with pressure transducer.
• Aeration Basin - 4 cells 200' x 28' x 14' SWD each. 15 hours detention time at 3.75 MGD.
• Final clarifiers - Three 60' diameter by 10' SWD with combined overflow rate of 442
GPD/SF at 3.75
• Chlorine contact basin - 40 minutes detention time at 3.75 MGD.
• Effluent pumps - 3 vertical turbine pumps. One pump rated at 6.5 MGD. Two pumps
running yield 7.5 MGD capacity.
As wastewater solids concentrations rise in the aeration basin a small portion of the sludge
recirculation flow, or approximately 150 GPM is routed to the sludge handling facilities. Details of the
major components of the sludge handling facilities are as follows:
• Small high rate dissolved air floatation thickener which quickly thickens the waste sludge
from 1 % to 5% solids.
• Aerobic digester-150,000 gallon capacity.
• Secondary aerobic digester/sludge holding tank — 500,000 gallon capacity.
• Sludge drying beds six at 1,600 s.f. each

The Division is beginning the cycle of permit renewals for the Cape Fear river basin. An examination of the Cape
Fear basin plan and the NPDES permit schedule has revealed that the existing expiration dates in Cape Fear NPDES
permits do not coincide with the revised permit expiration dates for the basin. This discrepancy could create unnecessary
delays in the processing of over 200 permit renewals. Accordingly, the Division is changing the permit expiration dates for
NPDES permits in the Cape Fear river basin.
This permit modification changes the expiration date of the subject permit in subbasin 30613 to September 30, 2001.
Please find enclosed the revised permit cover page. Insert the new cover page into your permit and discard the old page.
All other terms and conditions contained in the original permit remain unchanged and in full effect. This permit
modification is issued under the requirements of North Carolina General Statutes 143-215.1 and the Memorandum of
Agreement between North Carolina and the U. S. Environmental Protection Agency.
In accordance with the modification of the permit expiration date, the new due date for your renewal application is
April 3, 2001. Renewal application forms will be sent to you approximately 6-8 weeks in advance of the new due date or
you may download the forms from the NPDES web site at http://h2o.enr.state.nc.us/NPDES/documents.html. You
will need to download Standard Form A. If you have already submitted a permit renewal form, there is no need to
submit any other forms to the Division; the application will be processed during the renewal period for your subbasin.
If any part of this permit modification are unacceptable to you, you have the right to an adjudicatory hearing upon
written request within thirty (30) days following receipt of this letter. This request must be a written petition conforming
to Chapter 150B of the North Carolina General Statutes, filed with the Office of Administrative Hearings (6714 Mail
Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding.

to receiving waters designated as the Cape Fear River in the Cape Fear River Basin
in accordance with effluent limitations, monitoring requirements, and other
conditions set forth in Parts I, II, III and IV hereof.
This permit shall become effective September 1, 2000.
This permit and authorization to discharge shall expire at midnight on September 30,
2001.
