North Carolina General Statute 143-215.613 (i) provides that: Any person who knowingly makes any false

required to be maintained under this Article or a rule implementing this Article ... shall be guilty of a Class 2
misdemeanor which may include a fine not to exceed ten thousand dollars ($10,000).

We have received your letter dated June 12, 2020 and received in our office June 15, 2020 regarding
representative outfall status at Greenheck Fan Corporation located in Shelby, North Carolina. This letter
requests representative outfall status for the following Stormwater outfalls:
• Outfalls #2 and #3 as representative of Outfall # 1
The Mooresville Regional Office staff inspected the subject facility on June 25, 2020. During the
inspection we reviewed analytical monitoring data and viewed all of the outfalls on site. After completing
the inspection and reviewing the analytical monitoring data, we have concluded that the above
Representative Outfall Request will effectively represent the activities of the drainage area.
Please retain a copy of this letter in the Stormwater Pollution Prevention Plan for your records. If you

Based upon inspection conducted, representative outfall status is recommended for outfall one. Outfall one recieves
stormwater only from the employee parking area. Nothing was being stored and there were no other discharge sources in
