Division of Water Resources' Central and Regional staff has reviewed the application package
received November 5, 2021. However, additional information is required before the review may be
completed. Please address the items on the attached pages no later than the close of business on February
11, 2022.
Please be aware that you are responsible for meeting all requirements set forth in North Carolina
rules and regulations. Any oversights that occurred in the review of the subject application package are
still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items
in Sections A through D, or failure to provide the additional information on or before the above requested
date may result in your application being returned as incomplete.

A. Groundwater Monitoring Wells:
The current upgradient monitoring well MW-1 is located on the opposite side of a low-lying area
that collects water and drains to an on -site pond. This well does not represent the groundwater that
could be impacted by the irrigation system. MW-1 should be permanently abandoned, and a Well
Abandonment Record (Form GW-30) shall be completed and submitted. A new upgradient
monitoring well needs to be installed as it has been required since 2008 and a Well Construction
Record (Form GW-1) shall be completed and submitted. An updated site map needs to be
submitted.
B. Residuals Manaeement Plan:
1. A sludge survey needs to be completed. Sludge has not been removed from the lagoons for the
lifetime of the system. The ORC asked for methods of determining the amount of sludge in the
lagoons. During the March sampling visit, the ORC stated that there are often solids sprayed before
the effluent clears up.
C. Operation & Maintenance:
1. Please have the sump pump installed in the pumphouse.
2. It was noted in the December 20, 2021 site visit that the drain from the manure capture area is now
piped into the lagoons. The concrete pad that collects manure and other leachate is not cover and
could introduce stormwater to the lagoons. It is unclear whether the leachate is piped directly into
the lagoons or if it goes through the bar screen. Was the pipe part of the original engineering design?
If not, when was the pipe installed? Was the regional office or the central office staff notified?
D. Site Map:
1. Please provide an updated the site map addressing the following comments:
a. The lagoons are not outlined, they are just labeled. The topography labels make it hard to see
the lagoons.
b. The compliance and review boundaries are incorrect as shown. The compliance boundary is
defined by the permit as 500 ft from the effluent disposal area, or the property boundary,
whichever is closer. The review boundary is midway between the compliance boundary and
the effluent disposal area. There are several areas that needs to be reassessed.
c. Distance measurements ensuring all setbacks are being met are not shown.
d. Habitable residences and places of assembly within 500ft of all waste treatment, storage, and
disposal sites aren't labeled. There may be a habitable residence on the eastern side of the
property that is within 500 ft of the spray zones.
e. The pond on the property is not labeled and the stream line is not included on the legend of the
map.
£ The bar screen and grease trap are not shown on the map.
g. The map shows a different number of risers in Zone 4. All material that Ms. Caudle and Mr.
Moore have shown three risers.
