Directions: Print or type all entries on this certification form. Attach this form and a copy of the original ROS
approval documentation to the Stormwater Upload Form.
North Carolina General Statute 143-215.6B (i) provides that: Any person who knowingly makes any false
statement, representation, or certification in any application, record, report, plan, or other document filed or
required to be maintained under this Article or a rule implementing this Article ... shall be guilty of a Class 2
misdemeanor which may include a fine not to exceed ten thousand dollars ($10,000).
Under penalty of law, I certify that:
I, as an authorized representative, request renewal of the ROS designations currently granted to the subject
facility.
✓❑ No site or activity modifications that impact the characteristics of stormwater discharges from the
representative outfalls have occurred at the subject facility.
I am familiar with the information contained in this request and to the best of my knowledge and belief such
information is true, complete, and accurate.

Dear Mr. Ferguson:
Based on information provided in your letter received May 8, 2002, and the drainage areas that were
delineated on the enclosed site map, it has been determined that the outfalls you described should sufficiently
represent your waste dumps and mining activities. Therefore, monitored discharges from outfall 110 for the
waste dumps and outfall 107 for the mining areas are acceptable. One area that your letter neglected however
was your plant processing area. A subsequent conversation with yourself on May 6, 2003 confirmed that
outfall 116 would be monitored as representative of this area. As also stated in your letter, outfall 002 will
continue to be monitored for mine dewatering discharges.
Also enclosed with this letter is the $80 check that you submitted for a minor modification since there
is no fee associated with obtaining representative outfall status.
Please note that qualitative monitoring is still required at all outfalls at the facility regardless of
representative outfall status, as stated in Part III, Section B, Section 3 of General Permit NCG020000.
