MINUTES AT 16 MINUTE INTERVALS DELIVERING A DOSE OF 2400 GALLONS/DAY OF SEPTIC TANK
EFFLUENT. TREATED EFFLUENT LEAVING THE E-Z TREAT UNIT SHALL BE DIRECTED TO A FLOW
SPLITTER (75% BACK TO THE E-Z TREAT RE-CIRC TANK, 25% FORWARD TO THE FIELD DOSING
TANK).
THE DRIP DISPERSAL SYSTEM SHALL BE CONTROLLED BY A MICROPROCESSOR WHICH
ACTIVATE THE PUMP AND SEQUENCING OF THE APPROPRIATE FIELD SOLENOID VALVE,
PRESET, VARIABLE, TIMER TO DELIVER 25 GALLONS TO EACH OF THE FOUR ZONES.
SHALL BE REPEATED AT 4 HOUR INTERVALS PROVIDED THE DOSE ENABLE FLOAT IS A
IF THE HIGH WATER ALARM FLOAT SWITCH IS ACTIVATED, THE SYSTEM WILL ENERGIZE

DEFLECTION TESTING
THE DEFLECTION TEST SHALL CONSIST OF MEASURING THE PIPE FOR VERTICAL RING DEFLECTION.
MAXIMUM RING DEFLECTION OF THE PIPELINE UNDER LOAD SHALL BE LIMITED TO 5 PERCENT OF
THE INTERNAL PIPE DIAMETER. ALL PIPE EXCEEDING THIS DEFLECTION SHALL BE CONSIDERED TO
HAVE REACHED THE LIMIT OF ITS SERVICEABILITY AND SHALL BE RELAID OR REPLACED BY THE
CONTRACTOR AT THE CONTRACTOR'S SOLE EXPENSE.
THE DEFLECTION TESTING SHALL BE ACCOMPLISHED BY PULLING A MANDREL, SPHERE, PIN -TYPE
"GO/NO GO" DEVICE THROUGH THE PIPELNE. THE DIAMETER OF THE MANDREL, SPHERE, "GO/NO
GO" DEVICE SHALL BE 95 PERCENT OF THE UNDEFLECTED INSIDE DIAMETER OF THE FLEXIBLE
PIPE.
PRESSURE AND LEAK TESTING
GENERAL
1. PRIOR TO TESTING ANY SEGMENT OF FORCE MAIN, CARE SHALL BE TAKEN TO PREVENT THE PIPE FROM
MOVING WHILE UNDER PRESSURE.
2. TEMPORARY TAPS AND AIR RELEASES SHALL BE PERMISSIBLE TO FACILITATE TESTING.
3. WATER USED FOR TESTING FORCE MAIN INSTALLATIONS SHALL EITHER BE DISPOSED IN A NEARBY
SANITARY SEWER, AS AUTHORIZED BY THE LOCAL SEWER AUTHORITY, OR IN ANOTHER LOCATION IN
ACCORDANCE WITH STATE AND FEDERAL LAWS AND REGULATIONS.
4. ALL TESTING SHALL BE PERFORMED IN THE PRESENCE OF THE APPLICANT, THE PE, OR OTHER
AUTHORIZED REPRESENTATIVE.
5. THE RESULTS OF ALL TESTING SHALL BE MAINTAINED BY THE APPLICANT AS PART OF THE CONSTRUCTION
RECORD DOCUMENTATION.
PRESSURE TESTING
1. A HYDROSTATIC PRESSURE TEST SHALL BE PERFORMED ON EACH SEGMENT OF INSTALLED FORCE MAIN.
2. THE TEST SHALL BE PERFORMED AFTER THE FORCE MAIN HAS BEEN BACKFILLED AND AT LEAST SEVEN
DAYS FOLLOWING THE POURING OF THE LAST THRUST BLOCK.
3. THE FOLLOWING PROCEDURES SHALL BE FOLLOWED IN PERFORMING HYDROSTATIC PRESSURE TESTS ON
FORCE MAINS:
A. THE FORCE MAIN SEGMENT SHALL BE CAREFULLY FILLED WITH WATER AT A VELOCITY OF APPROXIMATELY
THE FORCE MAIN SHALL BE TAKEN DURING THIS PROCESS.
ONCE FULL OF WATER, THE FORCE MAIN SEGMENT SHALL BE PRESSURIZED AND ALLOWED TO STABILIZE
AT A TEST PRESSURE OF 5 POUNDS PER SQUARE INCH.
B. THIS PRESSURE SHALL BE MAINTAINED FOR AT LEAST ONE MINUTE PER 100 FEET OF LINE.

The E—Z Treat treatment system is controlled by an Ohio simplex control panel which doses
the filter media for 3 minutes every 16 minutes. Field dosing will be controlled with a E—Z
Treat one —zone Controller. The Controller will activate the pump and associated contra) valves
to dose the irrigation zones every 4.0 hours. If less than 600 gallons are available, the low
level float will inactivate the pump.
If the high water level "alarm " float is activated, the system will begin its operation
of delivering the pre —determined dose volume and shall also energize the audible and flashing
alarm fixtures. Once energized, the flashing and audible alarm units may only reset manually.
When properly maintained and utilized, the E—Z Treat treatment system should meet the

The Controller will also control the periodic backwashing of the filters and flushing operation
of the sub —field lateral network. Filter backwashing shall occur at the end of each dose
cycle or by recognition of a pre—set elapsed time since last system flush.
System lateral flushing shall occur when the microprocessor senses recognition of a pre—set elapsed
time since last system flush (in this case, at least 50 cycles) and after a power outage. The system shall
be configured to allow for manual initiation of backwashing filters or sub —field system flushing.
For this system, the flushing time shall equal the zone dose time of approximately 8.3 minutes
and shall occur every 8 days (1% of the pump run time in each zone).
The controller may log and record, for later retrieval or transmission, the times of operating
cycles, gallons dosed by zone, and any alarm condition.
The main pump control panel shall include, but not be limited to an " auto / manual " system
control switch, stop /start switch for each pump, and individual pump run lights.
The main pump control panel shall be a NEMA 4x enclosure. All supporting control units, if not
housed within the main pump control panel, shall be in a seperate NEMA 4x enclosure.

USE PERMANENT SILICONE OR EXPANDABLE FOAM PRODUCTS.
11. ALL PUMP AND FLOAT CONTROL CORDS SHALL BE SUPPLIED WITH EITHER 30 FT. OR 50
FT. LEADS. THIS WILL HELP KEEP SPLICES TO A MINIMUM, WHICH AND SHOULD BE AVOIDED IF
POSSIBLE. IF A SPLICE MUST BE MADE, IT MUST BE MADE ABOVE GRADE IN A NEMA 4X
JUNCTION BOX. NO SPLICES SHALL BE MADE IN ANY TANK.
12. ALARMS SHALL BE AUDIBLE AND VISUAL. ALARMS SHALL ACTIVATE THE WIRELESS AUTO DIALER

The Permittee agrees to operate and maintain the single-family residence wastewater treatment and irrigation system
as follows:
1. Inspect the septic tank annually, and pump out solids as needed.
2. Inspect and clean the septic tank effluent filter annually. (Li applicable)
3. Inspect the tablet chlorinator weekly. Add wastewater grade chlorine tablets (e.g., calcium hypochlorite) as
needed. Swimming pool grade chlorine tablets are not acceptable. (if applicable)
4. Inspect the ultraviolet disinfection unit weekly. Clean or replace the lamps and quartz sleeves as needed. (if
applicable)
5. Inspect all storage tanks, pumps, and alarms monthly. Remove the floating scum layer in all pump/storage tanks
when pumping the septic tank solids out.
6. Inspect the spray irrigation system monthly to verify: proper operation of the spray heads; that there are no leaks;
that vegetative growth does not obstruct the spray heads: that the irrigated wastewater is not ponding in or -
running off the designated irrigation area: and that there are no objectionable odors. (if applicable)
7. Inspect the drip irrigation system monthly to verify: proper operation of the drip lines; that there are no leaks; that
vegetative growth does not obstruct the drip emitters: that the irrigated wastewater is not ponding in or running
off the designated irrigation area; and that there are no objectionable odors. (if applicable)
8. Maintain a set of Division-approved engineering plans and specifications.
9. Pay the required annual fee.
10. Request renewal of this permit on Division-approved forms no later than 180 days prior to expiration.
11. Sign and provide a Change of Ownership application to any future owner of the single-family residence
wastewater treatment and irrigation system for their completion and submission to the Division of Water
Resources.
