In accordance with your application to discharge under the subject General Permit, the Division of Water
Resources (DWR) hereby forwards this Certificate of Coverage (COC). We issue this certificate pursuant
to the requirements of North Carolina General Statue 143-215.1 and the Memorandum of Agreement
between North Carolina and the US Environmental Protection agency dated October 15, 2007 (or as
subsequently amended). The following information is included with your permit package:
• A copy of the Certificate of Coverage for your treatment facility
• A copy of General Wastewater Discharge Permit NCG500000
If any parts, measurement frequencies or sampling requirements contained in this general permit are
unacceptable, you have the right to request an individual permit by submitting an individual permit
application. Unless such demand is made, the certificate of coverage shall be final and binding.
Please note that this Certificate of Coverage is not transferable except after notice to the Division of
Water Resources. The Division may require modification or revocation and re -issuance of this COC. This
permit does not affect your legal obligation to obtain other permits that may be required by the Division
of Water Resources, the Division of Land Resources, the Coastal Area Management Act, and/or any other
Federal or Local governments.

To discharge cooling waters associated with cooling tower blowdown, boiler blowdown,
air compressor condensate, and similar wastewaters under the
NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM
(NPDES)
In compliance with the provisions of North Carolina General Statute 143-215.1, other lawful
standards and regulations promulgated and adopted by the North Carolina Environmental
Management Commission, and the Federal Water Pollution Control Act, as amended,
Horsehead Metal Products, Inc.
is hereby authorized to discharge wastewater from a facility located at
484 Hicks Grove Road, Mooresboro 28114
Rutherford County
to receiving waters designated as the Broad River [stream segment [9-(36.5)], a waterbody currently
classified C within Subbasin 03-08-02 of the Broad River Basin, in accordance with effluent limitations,
monitoring requirements, and other conditions set forth in Parts I, II, III and IV of the General Permit
NCG500000, as attached.
This permit shall become effective February 7, 2014.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

I appreciate all of your help with this. I met with Jim Harris of Horsehead yesterday and one item that we would like to
address is the name of the permit holder. On the application, we had Horsehead Metal Products, Inc. c/o Anthony
Staley but the permit was issued to the Horsehead Holding Corporation c/o Darin Cooper. I assume this is because that
is the name on the other discharge permits. We would like to take the steps to modify all of the permits to Horsehead
Metal Products, Inc. Can you guide me into what steps we need to take to accomplish this?

We've been part of the conversation and have been to the site.
Sergei is intimate with the situation as well.
We all thought we could get the WW to be covered by the general permit into the individual, but the flow would push
the permittee into a major and they don't want that. So, stuck with two permits.
Let me know if we can provide any info or what not.

We have been getting calls on the status of Horsehead Metal Products, Inc. COC NCG500667 for Boiler & Cooling Tower
Blowdown and Air Compressor Condensation. I've had this one less than two weeks, so this one came in
hot. Apparently, it has an internal outfall with an individual permit NC0089109. I was wondering if someone has already
looked at this one along with the review for NC0089109. Hope you all are staying warm up there in this weather.

a POTW is not an option. It should also include a present value of costs analysis as outlined in the Division's
"Engineering Alternatives Analysis (EA.-1) Guidance Document."

Additional Application Requirements:
For new or proposed discharges, the following information must be included in triplicate with this application or it
will be returned as incomplete. Per 15A NCAC 021-1 .0105(c)
a) 7.5 minute series USGS topographic map (or a photocopied portion thereof) with discharge location clearly
indicated.
b) Site map, if the discharge is not directly to a stream, the pathway to the receiving stream must be clearly
indicated. This includes tracing the pathway of a storm sewer to its discharge point.
c) If this application is being submitted by a consulting engineer or engineering firm), include documentation
from the applicant showing that the engineer (or Firm) submitting the application has been designated an
authorized Representative of the applicant, per 15A NCAC 021-I .0138(b)( I ).
d) Final plans for the treatment system (if applicable). The plans must be signed and sealed by a North
Carolina registered Professional Enuineer and stamped — "Final Design -Not released for construction," per
I5A NCAC 021-1 .0139.

Any person who knowingly makes any false statement representation, or certification in any application, record, report,
plan, or other document filed or required to be maintained under this Article or a rule implementing this Article; or who
knowingly makes a false statement of a material fact in a rulemaking proceeding or contested case under this Article; or
who falsifies, tampers with, or knowingly renders inaccurate any recording or monitoring device or method required to be
operated or maintained under this Article or rules of the Commission implementing this Article, shall be guilty of a Class 2
misdemeanor which may include a fine not to exceed ten thousand dollars (S10,000). I S U.S.C. Section 1001 provides a
punishment by a fine or imprisonment not more than 5 years, or both, for a similar offense.

13. a) This connection is not possible due to a combination of the overall flow rate and the lack of BOD.
The Town of Forest City did not want to accept this discharge. It is their opinion that this flow
would render the package WWTP inoperable.
b) Subsurface disposal is not practical due to the low available acreage, steep slopes, proximity to

cooling tower and air compressor blow downs, the Town of Forest City strongly encouraged
Horsehead to seek an alternate source of discharge as opposed to releasing this in the Town's
