1) The provided coordinates for the proposed well SA-1 are not correct, and this well
feature also appears to be missing from the provided site map titled "Proposed
Mulberry WWTP Overall" (the map only depicts two proposed wells near Field
#14). Please provide the correct coordinates for the proposed wells. For reference,
these are the provided coordinates from the most recently revised application form:

The application sheet has been revised with corrected well coordinates and figure revised. See
attachments 1 and 2.
2) Please confirm that the existing chlorine disinfection system located west of Lagoon
3 is capable of meeting the fecal coliform requirements while operating under the
new lagoon configuration. Were calculations performed to assess the existing
disinfection design?
The new lagoon configuration provides for additional settling but no change to treatment.
The existing disinfection contact is not altered. Therefore, no calculations have been
performed.
3) Within the revised calculations for Phosphorus loading, we noticed that the total
phosphorus values from PPI 001 and PPI 002 were averaged using an ordinary
arithmetic mean. Can you please explain why this method was chosen over a
weighted arithmetic mean approach that would address the differing flow
conditions between the two sampling points?
We have reviewed and revised the calculations for phosphorous loading using weighted
average of the two existing lagoon influent locations. The revised average concentration is
4.52 mg/L of phosphorous to the new spray field. The revised calculations showing suitable
spray area land for the updated phosphorus loading are provided in Attachment 3.

4) Have the easements for the influent force main been acquired? Please note that not
having the easements will change some of the language and conditions within the
draft permit.
The easements are still in the process of acquisition.
We appreciate the comments received and your assistance with this project.
Sincerely,

IV. GENERAL REQUIREMENTS —15A NCAC 02T .0100 (continued):
8. What is the nearest 100-year flood elevation to the facility? 25.2 feet mean sea level. Source: Flood Risk Information
System, Nearest to the spray irrigation is a Zone A flood plain. Nearest to the existing Shallotte WWTF is a Zone AE flood
plain with a base flood elevation of 25.2 ft.
Are any treatment, storage or irrigation facilities located within the 100-year flood plain? ❑ Yes or ® No
y If yes, which facilities are affected and what measures are being taken to protect them against flooding? A small
LiLon of the proposed spray irrigation is located in a Zone A flood plain. There are many irrigation spray fields that serve
the Shallotte WWTF. The total capacity of these spray fields is enough that not every spray field is needed everyday.
Therefore should the proposed sprav irrigation field flood. the effluent can be diverted to other irrigation sprav fields.
If yes, has the Applicant submitted written documentation of compliance with & 143 Article 21 Part 6? ❑ Yes or ❑ No
9. Has the Applicant provided documentation of the presence or absence of threatened or endangered aquatic species utilizing
information provided by the Department's Natural Heritage Program? ® Yes or ❑ No
10. Does the facility have a proposed or existing groundwater monitoring well network? ® Yes or ❑ No
If no, provide an explanation as to why a groundwater monitoring well network is not proposed:
If yes, complete the following table (NOTE — This table may be expanded for additional wells):
