The sewage and wastewater collected by this system shall be treated in the City of Salisbury WWTP (NPDES No. NC0023884) prior to being discharged into the receiving stream. If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150B of North Carolina General Statutes, and filed with the Office of Administrative Hearings, 6714 Mail Service Center, Raleigh, NC 27699-6714. In the 1st page Under the consultant information PE License number is invalid "F0177" could you please update . 2. Under the flow calculations I see the homes listed at 240 GPD, are those going to be 2 bedroom homes? 3. Since this is a modification, wanted to clarify is this all additional to what was previously permitted, 3143 If of 8in If of gravity and 32,000 GPD? 4. The flow tracking form has the PS information empty, wanted to confirm that there are no PS in the way and that it was not left out.
