The above referenced project was inspected by Tyler MacConnell on three separate occasions
under my supervision with the last visit being on February 23, 2022. 1 certify that the above
referenced project was decommissioned with due care and to acceptable construction standards as
per the attached closure plan. I trust that this letter meets your requirements for proper
decommissioning under NC General Permit NCG550000.

Closure Plan for NPDES Discharge System Covered Under
NC General Permit NCG550000
Han -Dee Hugo's (Sampson Bladen Oil)

Recommended On -Site Wastewater System Abandonment Procedures.
1. Have an approved hauler pump the liquid, sludge/septage and scum from all tanks in the system.
Remove the tanks or crush them in place. If crushed in place, broadcast crushed tank with lime to
disinfect. Backf ll the excavation to a natural grade and establish a vegetative cover.
2. Disconnect power at the source to all electrical controls and remove all controls and panels.
3. Disconnect all non -tankage equipment. Pressure wash and disinfect with bleach solution those
surfaces which come in contact with wastewater, prior to removing from site.
4. Remove all parts of the discharge system (such as pipe, valves, valve boxes and risers) backfill
the area to a natural grade and establish a vegetative cover.
5. Coat all surface areas exposed to wastewater/effluent with hydrated lime and establish a
vegetative cover.
6. Wait at least 18 months before using the disposal area for gardening or construction.
