In accordance with your application, the Division of Water Quality (DWQ) hereby forwards this Certificate of
Coverage to discharge under the subject general permit. We issue this permit pursuant to the requirements of
North Carolina's General Statue 143-215.1 and the Memorandum of Agreement between North Carolina and
the US Environmental Protection Agency dated July 17, 2007 (or as subsequently amended).
The following information is included with your permit:
• A copy of the Certificate of Coverage for your treatment facility
• A copy of General Permit NCG500000 for boiler blowdown discharge
• A copy of a Technical Bulletin for General Permit NCG500000
If any parts, measurement frequencies or sampling requirements contained in this general permit are
unacceptable, you have the right to request an individual permit by submitting an individual NPDES permit
application. Unless such demand is made, this Certificate of Coverage shall be final and binding.
Please note that this Certificate of Coverage is not transferable except after notice to the Division of Water
Quality. The Division may modify, or revoke and reissue this Certificate of Coverage. This issuance does not
affect your legal obligations to obtain other permits that may be required by the Division of Water Quality, the
Division of Land Resources, the Coastal Area Management Act, or any other Federal or Local government.

This permit shall become effective June 6, 2011.
This Certificate of Coverage shall remain in effect for the duration of the General Permit.

Dear Mr. Matthews:
S&ME, Inc. (S&ME) is pleased to present the attached application for Boiler Blowdown
Discharge coverage under general permit NCG50. S&ME has been retained by
Butterball to provide consulting services and to prepare the attached Notice of Intent and
associated documents for the boiler blowdown discharges.
The existing facility is being expanded to produce pelletized feed for poultry farms. As
currently understood by S&ME, these discharges are regulated under the NCDENR
Division of Water Quality General Permit No. NCG50. Boiler blowdown discharges will
be discharged cyclically as a part of routine processes for the Butterball facility. The
discharge is estimated to be released at a rate of 4,560 gallons per day. This discharge
will be conveyed through a stormwater ditch to an existing dry detention basin. From
there flows discharge to the north side of the CSX railroad and continue to the west to
ultimately discharge into Six Runs Creek (see Figure 1 and included site drawings
prepared by the site designer WL Port -Land Systems). Based on USGS low flow studies,
the 7Q10 flow for Six Runs Creek is 0.2 cfs.
Feedwater treatment consists of two products, SELoxy 628 and SELplus 732M, both
products from Southeastern Laboratories, Inc. MSDS sheets have been included in
Appendix I along with daily system balance projections and completed BioCide 101
forms. Neither product has been assessed for LC50 or EC50 values by Southeastern
nip Laboratories as the products are not labeled as biocides.
A connection to POTW is not possible for discharge as there is no sanitary sewer service
■* provided to this portion of Sampson County. Furthermore, existing soils on the site had

previously been overlaid with 3 to 5 feet of fill, preventing the possible discharge of a
wastewater stream of this magnitude to a conventional subsurface system. S&ME
prepared a present value cost estimate for a spray irrigation disposal system which is
provided in Appendix II.
A copy of the executed AS-071 contract and associated Scope of Services has been
provided in Appendix III serving as authorization for S&ME to prepare this Notice of
Intent. Also enclosed is a check in the amount if $100 for the permit fee. Please feel free

outlined in the Division's "Guidance For the Evaluation of Wastewater Disposal Altematives`.
15) Additional Application Requirements:
For new or proposed discharges, the following information must be included in triplicate with this
application or it will be returned as incomplete.
a) 7.5 minute series USGS topographic map (or a photocopied portion thereof) with discharge location
clearly indicated.
b) Site map, if the discharge is not directly to a stream, the pathway to the receiving stream must be
clearly indicated. This includes tracing the pathway of a storm sewer to its discharge point.
c) If this application is being submitted by a consulting engineer (or engineering firm), include
documentation from the applicant showing that the engineer (Or firm) submitting the application has
been designated an authorized Representative of the applicant.
d) Final plans for the treatment system (if applicable). The plans must be signed and sealed by a North
Carolina registered Professional Engineer and stamped -"Final Design -Not released for construction".

assumes a three-foot wide trench that has a maximum length of 100 feet, and 6

Dear Mr. Mandella:
S&ME, Inc. (S&ME) is pleased to provide this scope of work and budget estimate to
provide engineering services for WL Port -Land Systems, lnc. (WL Port -Land) for
environmental permitting and wastewater treatment consulting services. The request for
this scope of work resulted from a phone conversation with Mr. Bruce Mandella of WL
Port -Land and Mr. Lankford Ruffin of Butterball on February 23, 201 1 with Joshua Bell
of S&ME. The proposed scope of work and budget estimate are summarized in the
remainder of this letter.
BACKGROUND
WL Port -Land is working with Butterball to expand the facilities in Turkey, North
Carolina. As part of this expansion, boiler blowdown discharges will be discharged
cyclically as a part of routine processes. As currently understood by S&ME, these
discharges are regulated under the North Carolina Department of Environment and
Natural Resources Division of Water Quality General Permit No. NCG500000.
Additionally, we understand this project is to provide technical assistance with
determining a permitting option for an approximate 1,800 gallon per day (gpd) flow of
wastewater generated by a proposed truck disinfectant truck wash. S&ME was requested
to provide consulting services and design for the General Permit application for the boiler
blowdown discharges and for assessing the onsite truck wash wastewater disposal
system.
SCOPE OF WORK
S&ME proposes to perform the following tasks to engineering services to WL Port -Land
for the Butterball facility:

Part 1- Boiler Blowdown General Permit Application
Boiler blowdown discharges will be discharged cyclically as a part of routine
processes for the Butterball facility. As currently understood by S&ME,'these
discharges are regulated under the NCDENR Division of Water Quality General
Permit No. NCG500000. S&ME proposes to prepare the Notice of Intent
permitapplication. form to be submitted to NCDENR. As part of this
appli?ation, S&ME will summarize the infbrnmation obtained from WL Port -
Land regarding the boiler blowdown distil arge and prepare figures to show the
location of the discharge, storm water controls on site, and downstream
receiving water bodies and their classifications.
Part 2 - Preliminary Wastewater System Evaluation
We understand the purpose of this project is also to provide technical assistance
with determining a permitting option for an approximate 1,800 gallon per day
(gpd) flow of wastewater generated by a proposed truck disinfectant truck wash.
The initial task will be to determine a regulatory accepted design flow and to
perform a preliminarily soil evaluation to determine the soil suitability of on -site
soil areas available for wastewater disposal. The preliminary soil evaluation
will be performed in regard to the permitting of on -site subsurface wastewater
treatment :and'd!sposal systems (septic systems) permitted by the North Carolina
Department of Environment and Natural Resources (NCDENR), Division of
Environmental Health (DEH) and / or the permitting of surface wastewater
systems permitted by the NCDENR, Division of Water Quality (DWQ). At this
time potential wastewater flow to the areas to be evaluated has been
preliminarily determined to be approximately 1,800 gpd. Also, a definitive
system type (permitted by the DEH or DWQ) has not been determined and will
be determined based on findings associated with the preliminary soil evaluation
and further discussion with each of the regulatory agencies.
Task 2.1
S&ME proposes to assist with determining the proper permitting agency based
on wastewater characteristics and proposed wastewater flow. Determination of
which agency would be issuing a permit for this type of system, DEH or DWQ,
will be obtained.
S&ME proposes to assist with determining the potential design flow in gpd
associated with the proposed wastewater disposal system. Establishing the
proper design flow up front will be critical to determining system types along
with determining the permitting process associated with proposed system type.
Receiving Countyand/or. State concurrence during Task 2.1 for design daily
flow will need to beobtained prior to proceeding with additional services
associated with Task 2.3.
Task 2.2
S&ME proposes to evaluate areas identified by Butterball Turkeys (less than 5
total acres) for the potential permitting of on -site subsurface or surface

wastewater treatment and disposal systems using a phased approach. The
phases of this approach, which will be conducted in accordance with the "Laws
and Rules for Sewage Treatment and Disposal Systems (NC 15A NCAC 18A
.1900)", "Waste Not Discharged to Surface Waters (NC 15A NCAC 2T),
County Health Department guidelines, and current soil science practices and
technology, aredescribed in the following. sections.
Task 2 2.1— Preliminary Soil Scientist Evaluation
S&i $oil Scientists will conduct a site visit to evaluate soil conditions in the
identified areas to determine the feasibility of permitting of on -site surface or
subsurface wastewater treatment and disposal systems. Hand auger borings will:
be advanced at locations in the areas to be evaluated to provide the soil
scientists with sufficient information to evaluate the areas for on -site systems.
The hand auger borings will be evaluated for the following site specific
characteristics: texture; color; structure; soil depth; depth and thickness of any
restrictive horizon(s) if present; depth (if present) of a seasonal high water table;
and topography and landscape position. Hand auger borings will be located in
the field WWI a sub -meter GlpbalNosf ionipg System (C'PS). S&ME assumes
the Client will provide fbr utility dlearancc in the potential study areas.
Potential locations for the proposed on -site wastewater treatment and disposal
systems will be determined during Phase I.
Upon completion of the preliminary soil evaluation (Task 2.2.1), a map and
report of our findings will be prepared and submitted to the Client, and if
directed by the Client, the County Health Department and/or the NCDENR,
DEH and/or DWQ.
Task 2.2.2 — Site Visit with Regulatory Agencies
The County Health 'Department and/or NCDENR, DEH and/or DWQ will be
required to give the final approval for a proposed on -site subsurface or surface
wastewater treatment and disposal system location. As such, S&ME will meet
with the proper regulatory agency to verify the Preliminary Soil Scientist
Evaluation and to determine potential drainfield locations along with
determining a potential soil long term acceptance rate (LTAR) for the proposed
location/system. Preliminary regulatory concurrence will be sought at this point
and a determination of further evaluation for the permitting of the on -site
wastewater treatment and disposal system will be determined.
Part 3 - Future Wastewater System Design Scope
The scope andbudget estimate does not include septic system layouts,
applications to the county, D NQ or DEH, or further site meetings with agency
officials at this time. These services will be proposed on following the results
of the preliminary soils evaluation. Part 3 services will be determined upon
completion of Task 2.1 and 2.2. Additional services may include: Further Soils
Evaluation (if determined necessary during Task 2.2.2), proposed system
layouts, soil sampling to obtain soil chemical data, site loading capacity

analysis, assessment of groundwater impact, engineering design by a
Professional Engineer of the proposed systems, etc.
EXCLUDED SERVICES
Without attempting to be a complete list of all services or potential services that will be
excluded from this proposal but can be performed by S&ME, the following services are
specifically excluded from this proposal:.
• Detailed soil evaluations of each area referenced;
• The scope does not constitute or imply approval for permit as required by the
local health department. Soil evaluations are done based on interpretations of the
rules governing wastewater treatment and disposal systems and are not guarantees
for system approval. The local health department, DWQ, or DEH will need to
evaluate and permit each individual system;
• The scope does not include septic system layouts, applications to the county,
DWQ or DEH, or further site meetings with agency officials at this time. These
services will be proposed on separately following the results of the preliminary
soils evaluation;
• Detailed survey of the area of concern, and;
• Associated permit fees.
If any of the above excluded services are required, please contact us so that we can
modify this proposal or prepare a proposal for additional services.

client to execute the Parts defined in this proposal, the following services are specifically
required of the client:
• The Client is responsible for locating all utilities (electric, gas, water, etc) before

Note: This proposal is solely intended for the Basic Services as described above in the
Scope of Services. The Scope of Services may not be modified or amended, unless the
�-► changes are first agreed to in writing by the Client and S&ME. Use of this proposal and
corresponding final report is limited to the above -referenced project and client. No other
use is authorized by S&ME, Inc.

performed. Our estimate assumes that moderate to favorable conditions will be
encountered. If significantly different or adverse conditions are encountered, S&ME will
contact you concerning these conditions and discuss in detail any additional work that may
be necessary. S&ME will obtain written authorization prior to conducting any additional
work.

Any additional services outside of the proposed scope of services will be provided on a time
and expense basis according to the established fee schedule.
SCHsoUtE
Based upon staff availability, S&ME is prepared to initiate field activities within 5 to 7
business days upon receiving proper authorization. We anticipate the site visit and soil
evaluation will require approximately 1 business day to complete. The letter report can be
prepared within 3 to 5 business days from the date of completion of the site visit and soil
evaluation.
AUTHORIZATION
S&ME's signed Agreement for Services (Form Number AS-071) is attached and
incorporated as a part of this proposal. Please sign the last page of both copies of the
form, and return one copy to our office. Upon receipt of the signed agreement, we will
proceed with the performance of our services. Any changes or modifications to AS-071
or the proposal are required to be acknowledged by both parties initialing acceptance of
this proposal and agreement for services next to the change or modification. If you elect
to accept our proposal by issuing a purchase order, then please specifically reference this
proposal number and date. Your purchase order will be an acceptance of our Agreement for
Services and an authorization to proceed with theperformance of our services. The teims
and conditions included in any purchase order shall not apply, and are hereby specifically
rejected, as our agreement is for services which are not compatible with purchase order
agreements.
