Division 9
U-2579B UT to Fisher Branch (Forsyth) - Year 4
• ACOE- Agrees continue monitoring; site looks good to date.
• DWR- Asked for clarification/update re: slight bank erosion around PP#1.
o DOT states there has been no further change since the 2020 site visit.
Supplemental live stakes were planted Jan. 2021. Surviving at time of
inspection.
• DOT to continue monitoring in 2022
U-2707 UT to Muddy Creek (Forsyth) - Year 4
• ACOE- Site looks good to date.
• DWR-questioned Rosgen C5 design. Was this achieved?
o DOT -Yes. Designed and constructed per mitigation plan. There have
been several bankfull events throughout the monitoring period.
Channel monitoring throughout year 4 has revealed little change since
construction.
• DOT to continue monitoring in 2022
Division 12
1-3819A UT to Fourth Creek (Iredell) - Year 5
• DOT reposted report 3/14/2022 to correct the impacts noted in
introduction section of report.
• Silt loss from B section March 2021, Lane Construction removed
sediment and returned stream to original channel.
• Adjacent Development- sediment loss Jan. 2022.
o Site visit with agencies Feb. 14, 2022 to review offsite sediment
loss within mitigation area.
• DOT to continue monitoring in 2022
• EPA -Questioned if report would be updated.

o DOT updated the report to include intention to continue
monitoring on site. Revision was posted to EAU website
3/XX/2022
• DWR- Monitoring should be open ended until B section is stabilized. Also
report on adjacent development site.
• ACOE-good with comments.

R-2518A Middle Fork Creek Site I (Madison)
• Remediation underway. Update on schedule:
o Pre -bid — March 10
o Bid opening — March 16
o Availability —April 2022
• DOT will keep everyone in loop as construction progresses.
R-2519B Fox Hollow Creek Site 25 (Mitchell) - Year 2
• Head -cut discovered in 2020 not getting any worse to date.
• ACOE-no comments.
• DWR- Noted some new growth on multi -flora and Japanese knotweed.
• DOT - Continue to treat with herbicide.
• DOT to continue monitoring in 2022
R-2519B Long Branch Site 5A (Yancey) - Year 2
• DWR- Noted Multi -flora rose on site.
• DOT to continue monitoring in 2022
R-2519B Long Branch Site 6 (Yancey) - Year 2
• No comments
• DOT to continue monitoring in 2022
R-2519B Long Branch Site 8 (Yancey) - Year 2
• DOT- Herbicide treatment continues. Lespedeza will be mowed, sprayed
and seedlings replanted in 2022/2023.
• ACOE-No comments
• DWR-upper section treated?
o DOT -Yes. TOB to CA fence.
• DOT to continue monitoring in 2022
R-2519B Long Branch Site 9 (Yancey) - Year 2
• No comments
• DOT to continue monitoring in 2022

R-2519B Parsnip Branch Site 18 (Yancey) - Year 2
• DWR- Noted multi -flora rose on the site
• DOT- Herbicide treatment will continue, and DOT will investigate
invasives this growing season.
• DOT to continue monitoring in 2022
R-2519B UT to Big Crabtree Creek Site 21 (Yancey) - Year 2
• DWR- Replanting looks good. Noted multi -flora rose on the site
• DOT to continue monitoring in 2022
R-2519B UT to Brushy Creek Site 30 (Mitchell) - Year 2
• DWR-upper section 120'. Inquired about repair work.
o DOT -Profile remains similar since initial downcutting after
construction that was noted during first year monitoring. DOT will
consider placing boulder toe along 50'-60' of the left bank that
eroded early after construction.
• DOT will investigate and coordinate with agencies.
• ACOE-Reinforced that bank erosion needs to be fixed, if still exists.
Complete this summer, if possible.
• DWR-CA fence fell in creek upstream.
o DOT -This is above mitigation area per permit drawings.
• DOT to continue monitoring in 2022
R-2519B UT to Brushv Creek Site 33 (Mitchell) - Year 2
• No comments
• DOT to continue monitoring in 2022
R-2519B UT to Brushy Creek Site 35 (Mitchell) - Year 2
• DOT abandoned site after previous monitoring due to heavy infestation
of kudzu.
• ACOE-Questioned timing of permit modification. DOT to request permit
modification after completion of monitoring of remaining on -site
mitigation sites along the R-2519B project.

• DWR- Notified DOT of beaver presence on the site prior to today's
meeting.
• DOT- APHIS has removed beaver and dam first week of March 2022.
• DOT to continue monitoring in 2022
