Box 69 Middlesex, NC 27557 Subject: Compliance Evaluation Inspection Middlesex Collection System Deemed Permitted Collection System No. WQCSD0011 Nash County Dear Mr. Lewis: On March 17, 2022, Alys Hannum of the Division of Water Resources (Division) conducted an inspection of the Town of Middlesex's (Town) Collection System to evaluate compliance with the operation and maintenance of the subject deemed wastewater collection system permit. The cooperation and assistance of James Liles, Public Works Director, and William Lamm, Envirolink staff and ORC, during the inspection were greatly appreciated. The Town's Collection System is covered by a deemed permit (WQCD0011) and is classified as a CS-1 system. Deemed collection systems must meet the requirements of 15A NCAC 02T.0403. If the system ever exceeds 200,000 GPD, the Town would need to apply for an individual collection system permit. Findings during the inspection were as follows: 1. The wastewater collection system consists of 11 pump stations and 1 high priority aerial sewer line. Three of the 11 pump stations have permanent on -site generators, and all others have connections for portable generators. 2. The Town has developed and maintains a complete map of the collection system. Please ensure the map is updated to include: pipe material, pipe size, pump station names and capacity, and flow direction. 3. One sanitary sewer overflow to State Waters during the most recent 2-year period ending February 2022. The overflow was the result of an equipment malfunction. An operation and maintenance plan — including pump station inspection, preventative maintenance, and overflow response — was available and up to date. 5. The Town has a Fats, Oils, and Grease (FOG) program which includes the bi-annual distribution of educational materials to commercial and residential users. The Town is able to take enforcement against users who have not properly installed, operated, and/or maintained grease traps or grease interceptors, or who have otherwise violated the program. 6. Four of the eleven pump stations were inspected. All four pump stations had identification signs, secured access, appeared in good condition and were free of excess debris. All audible and visual alarms functioned when tested. The pump stations inspected were as follows: a. Nash Pump Station (#2) b. Hanes Pump Station (#4) c. White House Pump Station (#6) d. Mudcats Pump Station (#7) 7. A logbook for each pump station was available to view during the inspection. Pump station maintenance logs were reviewed and appeared satisfactory. Pump stations are inspected at least once a week by both the Public Works Director and the ORC. 8. The Town's single high priority aerial crossing line was inspected. It appeared straight, rust free, and in good condition. The right of way was being maintained.
