Chris — I just accessed the older COC renewal form to generate this one. Hopefully you got it, but I've attached that in
case you didn't get it. It looks like the 2018 renewal COC was never requested. It is now also stored in our repository

Suzanne, I have spoken with you previously for another location related to stormwater. I am now attempting
to gain access for United Plastics, my new employer, to access a Certificate of Coverage under the NPDES
program. It appears that United Plastics received a letter about this, but I can find no records or copies of the
Certificate of Coverage. When I go to the edocs website, it does not give me access without a username and
password that I do not have.
In order to print a copy of the COC and obtain a copy of the General Permit, can you assist me with access to
the online portal so that I can update information and print a copy of these?
