Dear Mr. Sheckells:
In accordance with the application received on March 3, 2022, and the additional requested
information received on March 16 & 18, 2022, we are forwarding herewith, Permit No. WS0500158 dated
March 21, 2022, issued for the construction of a water supply well network (five wells). This Permit will
be effective for one year from the date of its issuance and shall be subject to the conditions and limitations
as specified therein.
Please note this permit does not constitute approval of the volume of water proposed to be
withdrawn within an area defined as existing within a Capacity Use Area, or any permits issued by the local
health department. Additional permits and/or approvals may be necessary and we encourage you to contact
the appropriate agencies prior to constructing any supply wells. If you have any questions, please contact

PERMIT FOR THE CONSTRUCTION OF A WELL
In accordance with the provisions of Article 7, Chapter 87, North Carolina General Statutes, and other
applicable Laws, Rules and Regulations:
PERMISSION IS HEREBY GRANTED TO

FOR THE CONSTRUCTION OF A SUPPLY WELL SYSTEM (Sawyer's Mill S..D) consisting of five water supply
wells owned by Ten Oaks Realty LLC. at mailing address P.O. Box 257 Fallston, Maryland. The wells will be located
on the property owned by Patricia Lasater Duma & Debra L. Flentje at 8736 Bells Lake Road in Middle Creek, North
Carolina -Wake County. This permit is issued in accordance with the application received on March 3, 2022, and the
additional requested information received on March 16, & 18, 2022, in conformity with specifications and supporting
data, all of which are filed with the Department of Environmental Quality, which are considered a part of this permit.
This Permit is for well construction only, and does not waive any provision or requirement of any other
applicable law or regulation. This permit does not constitute approval of the proposed volume of water to be withdrawn,
within an area defined as a Capacity Use Area, overseen by the Division.
Construction of any well under this Permit shall be in strict compliance with the North Carolina Well
Construction Regulations and Standards (15A NCAC 2C .0100), and other State and Local Laws and regulations
pertaining to water supply well construction. Please be advised that some Counties may also require a permit.
This Permit will be effective for one year from the date of its issuance and shall be subject to other specified
conditions, limitations, or exceptions as follows:
I. Furnish copies of chemical analyses to the Division of Water Resources (DWR), Public Water Supply Section
(PWS), Raleigh Regional Office (RRO), 1628 Mail Service Center, Raleigh, N.C. 27699-1628.
2. Send the results of the aquifer test to the DWR, PWS, RRO 1628 Mail Service Center, Raleigh, NC 27699-
1628.
3. Please ensure the contractor responsible for constructing the well is a North Carolina certified water well
contractor.
4. It is necessary given the reported possibility of the presence of wetlands,etc. to take proper precautions during
well installationls to avoid any impact to surface water features that may result in a violation of the rules (15A
NCAC 02B)
5. Provide the Raleigh Regional Office, WQROS at least forth -eight (48) hours notice prior to the initiation of
well installation

a. All property boundaries, at least one of which is referenced to a minimum of two landmarks such as identified roads,
intersections, streams, or lakes within 500 feet of the proposed well or well system.
b. All existing wells, identified by type of use, within 500 feet of the proposed well or well system.
c. The proposed well or well system.
d. Any test borings within 500 feet of proposed well or well system.
e. All sources of known or potential groundwater contamination (such as septic tank systems, pesticide, chemical or fuel
storage areas, animal feedlots as defined in G.S. 143-215.108(5), landfills, or other waste disposal areas) within 500 feet
of the proposed well or well system.
2. As required by 15A NCAC 02C .0105(g)(3), for wells screened in multiple zones or aquifers, provide representative data on the
static water level, pH, specific conductance, and concentrations of sodium, potassium, calcium, magnesium, sulfate, chloride,
and carbonates from each aquifer or zone from which water is proposed to be withdrawn.
