Dear Mr. Crump:
The Division issued NPDES Permit NC0050342 to The City of Winston-Salem/Forsyth County Utility
Commission Muddy Creek WWTP on August 1, 2004. Your Permit Modification Letter dated March 7, 2006,
requested a change in location for your existing 002 outfall to Lat. 36° 58' 36", Long. 80° 20' 16". You indicated
that your facility would continue to utilize the current 002 outfall location until construction is complete on the new
outfall 002 (March 2007).
In accordance with our review, the Division is forwarding herewith a modification to the subject permit.
This permit modification is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and
the Memorandum of Agreement between North Carolina and the U. S. Environmental Protection Agency dated
May 9, 1994 (or as subsequently amended).
Please take notice that this permit is not transferable. This permit does not affect the legal requirement to
obtain other permits which may be required by the Division of Water Quality or permits required by the Division of
Land Resources, Coastal Area Management Act or any other Federal or Local government permit that may be
required.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are
hereby revoked. As of this permit issuance, any previously issued permit bearing this
number is no longer effective. Therefore, the exclusive authority to operate and discharge
from this facility arises under the permit conditions, requirements, terms, and provisions
included herein.
The City of Winston-Salem is hereby authorized to:
1. Continue operation of an existing 21.0 MGD wastewater treatment system that includes the following
components:
> Bar screens
> Grit chambers
> Primary clarification
> Three aeration basins
> Secondary clarification
> Chlorination and dechlorination
➢ Instrumented flow measurement
> Anaerobic sludge digesters
➢ Gravity thickener
> Sludge storage lagoons
> Belt filter press
> Solids blending tanks
This facility is located at the Muddy Creek WWTP off of Cooper Road near
Winston-Salem in Forsyth County.
2. Discharge from said treatment works through Outfall 001 located at Lat. 35 deg 55' 51", Long. 80 deg
22' 09" to the Yadkin River. The Yadkin River is currently classified WS-IV waters in the Yadkin -Pee
Dee River Basin.
3. Discharge from said treatment works through Outfall 002 located at Latitude 35 deg 58' 36", Longitude
80 deg 20' 16" to Muddy Creek. Muddy Creek is currently classified WS-IV waters in the Yadkin -Pee
Dee River Basin.

We are in receipt of your NPDES Permit Modification Letter dated March 20, 2006.
After reviewing your response we have the following concerns:
1. 1s1 paragraph of your cover letter indicates the existing 002 outfall at:
Latitude: 35 deg 58' 37".
The correct location for the existing 002 outfall is:
Latitude: 36 deg 59'37"
Longitude: 80 deg 22'53"
2. 2nd paragraph reads "Enclosed please find the modified permit effluent page."
No "effluent page" was included, only a revised "SUPPLEMENT TO PERMIT
COVER SHEET" was included.

3. Item 2 indicates the NEW 002 location to be:

NOTE: This is the current 002 location.
The correct NEW 002 outfall location will be:

Longitude: 80 deg 20' 16"
4. Item 2 no longer list the 001 discharge into the Yadkin River. This is a major
concern for us, as this is the PRIMARY effluent discharge location.
Existing 001 outfall:
Latitude: 35 deg 55'51"
Longitude: 80 deg 22'09"

Dear Mr. Guerra:
I am providing the following information after our conversation regarding the request for
permit modification dated February 14, 2006 to your office. As you have requested, I am
including the new coordinates for the proposed 002 discharge outfall location as well as
the current 002 outfall location.
The CURRENT 002 outfall is located at:
Latitude: 36 deg 59'37"
Longitude: 80 deg 22'53"
The NEW 002 outfall location will be:
Latitude: 35 deg 58'36"
Longitude: 80 deg 20'16"
As indicated in our conversation; Construction and Grants is currently reviewing this
project and has requested we apply for this permit modification as part of their approval
process. We anticipate going to bid on this project by the end of March 2006. Bid Award
will be in early May 2006 with a construction schedule for substantial completion taking
ten months (March 2007). We will continue to utilize the current 002 outfall location
until construction is complete on the new 002 outfall (March 2007). We will update you
on the progress approximately 30 days before we switch to the new 002 discharge
location.

The City of Winston-Salem asks that DWQ grant our request for a permit modification
allowing the 002 outfall to be relocated during this construction project and become
effective upon completion of the new effluent 002 location (anticipate March 2007).
