As requested the Division reviewed the proposed Muddy Creek Temporary Pump Station as well as the planned
addition of the South Fork Influent Diversion Box at the Muddy Creek WWTP NC0050342. Based on the
information provided, no compromise of the Muddy Creek WWTP operational integrity or minimum design
standards was noted. It was stated the on -site temporary pump stations operations will be retired in conjunction
with completion of the approved Authorization to Construct permit 050342A05. Therefore, the Division deems
the temporary pump stations and addition of the diversion box covered by the existing permit 050342A05, and
no further permitting is necessary.
A statement indicating the temporary pump stations were retired and the diversion box was installed is to be
written by the signing party on the Engineer Certificate form as confirmation these items were completed. I
recommend a copy of this email be attached to Winston-Salem CORRECTED Authorization to Construct
permit 050342A05 issued August 1, 2014 for future reference.

Dear Mr. Hargrove:
The initial letter of request for an Authorization to Construct was received March 26, 2014, by
the Division of Water Resources (Division), and the A to . C permit was issued July 3, 2014. The
Division was advised by Black & Veatch on July 29, 2014 that subsequent changes/corrections as the
result of the reduction in influent flow from the loss of Davie County wastewater, the availability of
alternative emergency power from the recently approved CHP project, and other noted corrections
were requested for the issued A to C permit 050342A05.
The submitted revised design, specifications, and corrections for the A to C permit 050342A05
project have been reviewed and revisions/corrections applied where appropriate in this CORRECTED
A to C permit 050342A05. The CHP project is independent of the A to C permit construction, and its
completion is anticipated prior to the completion of the A to C permit 050342A05 construction. As a
mandatory requirement to provide stand-by power for the additional electrical load, the 2,000 KW
diesel generator narrative must remain in the A to C permit. Upon completion of the CHP project and
connection to the Muddy Creek WWTP electrical grid, the CORRECTED A to C permit 050342A05
Engineer Certificate narrative should be edited by the signing party prior to submittal to the Division to
reflect the substitution of the CHP project for the listed stand-by diesel generator system.
Authorization is hereby granted for the construction of modifications as corrected to the
existing 21 MGD Muddy Creek WWTP, with discharge of treated wastewater into the Yadkin River
in the Yadkin -Pee Dee River Basin.

Please discard the original July 3, 2014 A to C permit 050342A05 and Engineer Certificate
form, and replaced it with this CORRECTED A to C permit 050342A05 and Engineer Certificate form
issued August 1, 2014.
This authorization results in no increase in design or permitted capacity and is awarded for the
construction of the following specific modifications:
Installation of new influent pump station consisting of three (3) 17,400 GPM influent
pumps, (2) 8,700 GPM influent pumps, two (2) 7,000 GPM transfer pumps, all with
variable speed drive, piping, and controls; installation of new headworks consisting of
three (3) 50 MG self-cleaning mechanical screens with three (3) screening wash/press
systems, two (2) 50 MG grit removal systems with four (4). 500 GPM grit pumps, four
(4) grit cyclones, two (2) grit separators, a bypass channel, area containment, piping, and
controls; installation of new primary clarifiers distribution box, piping, and controls;
installation of new plant drain pump station consisting of two (2) 1,000 GPM pumps, two
(2) 150 GPM pumps, piping, and controls; installation of two(2) new 1 MG EQ day tanks
with eight (8) 5 HP mixers (4 per tank), piping, and controls; installation of two (2) new
11,000 gallon Ferric Chloride storage tanks with containment, (4) 56 GPH chemical feed
pumps, piping, and controls; installation of new odor control system consisting of three
(3) carbon absorption units with three (3) 9,670 CFM fans, ductwork, and controls;
installation of new 4160V/3PH 2,000 KW diesel generator with 1,200 gallon double
wall fuel oil tank, piping, and controls; installation of new instrument air bubbler system
consisting of two (2) 2 HP air compressors, air dryer, piping, and controls; conversion of
two (2) existing lagoons to two (2) storm water EQ basins , approximately 3.85 MG
each, piping, and controls; conversion of one existing lagoon to a 4.2 MG centrate EQ
basin, piping, and controls; replacement of existing NPW pumps with lower capacity
pumps, two (2) new 400 GPM pumps, two (2) new 1,000 GPM pumps, piping, and
controls; in conformity with the project plans, specifications, demolition, and other
supporting data subsequently filed and approved by the Department of Environment and
Natural Resources.
This Authorization to Construct is issued in accordance with Part III, Paragraph A of NPDES
Permit No. NC0050342 issued with an effective date of September 1, 2009, and shall be subject to
revocation unless the wastewater treatment facilities are constructed in accordance with the conditions
and limitations specified in Permit No. NC0050342.
The sludge generated from these treatment facilities must be disposed of in accordance with
G.S. 143-215.1 and in a manner approved by the Division.
In the event that the facilities fail to perform satisfactorily, including the creation of nuisance
conditions, the Permittee shall take immediate corrective action, including those as may be required by
the Division, such as the construction of additional or replacement wastewater treatment or disposal
facilities.

The Winston-Salem Regional Office, telephone number (336) 771 - 5000, shall be notified at
least forty-eight (48) hours in advance of operation of the installed facilities so that an on site
inspection can be made. Such notification to the regional supervisor shall be made during the normal
office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays.
Upon completion of construction and prior to operation of this permitted facility, a certification
must be received from a professional engineer certifying that the permitted facility has been installed
in accordance with the NPDES Permit, this Authorization to Construct and the approved plans and
specifications. Mail the Certification to: NPDES Unit, DWR/DENR, 1617 Mail Service Center,
Raleigh, NC 27699-1617.
Upon classification of the facility by the Certification Commission, the Permittee shall employ
a certified wastewater treatment plant operator to be in responsible charge (ORC) of the wastewater
treatment facilities. The operator must hold a certificate of the type and grade at least equivalent to or
greater than the classification assigned to the wastewater treatment facilities by the Certification
Commission.
The Permittee must also employ a certified back-up operator of the appropriate type and grade
to comply with the conditions of T15A:8G.0202. The ORC of the facility must visit each Class I
facility at least weekly and each Class II, III and IV facility at least daily, excluding weekends and
holidays, must properly manage the facility, must document daily operation and maintenance of the
facility, and must comply with all other conditions of T15A:8G.0202.
A copy of the approved plans and specifications shall be maintained on file by the Permittee for
the life of the facility.
During the construction of the proposed additions/modifications, the permittee shall continue to
properly maintain and operate the existing wastewater treatment facilities at all times, and in such a
manner, as necessary to comply with the effluent limits specified in the NPDES Permit.
You are reminded that it is mandatory for the project to be constructed in accordance with the
North Carolina Sedimentation Pollution Control Act, and when applicable, the North Carolina Dam
Safety Act. In addition, the specifications must clearly state what the contractor's responsibilities shall
be in complying with these Acts.
Prior to entering into any contract(s) for construction, the recipient must have obtained all
applicable permits from the State.
Failure to abide by the requirements contained in this Authorization to Construct may subject
the Permittee to an enforcement action by the Division in accordance with North Carolina General
Statute 143-215.6A to 143-215.6C.
The issuance of this Authorization to Construct does not preclude the Permittee from
complying with any and all statutes, rules, regulations, or ordinances which may be imposed by other
government agencies (local, state, and federal) which have jurisdiction.

A letter of request for an Authorization to Construct was received March 26, 2014, by the
Division of Water Resources (Division), and final plans and specifications for the subject project have
been reviewed and found to be satisfactory. Authorization is hereby granted for the construction of
modifications to the existing 21 MGD Muddy Creek WWTP, with discharge of treated wastewater into
the Yadkin River in the Yadkin -Pee Dee River Basin.
This authorization results in no increase in design or permitted capacity and is awarded for the

wall fuel oil tank, piping, and controls; installation of new instrument air bubbler system
consisting of two (2) 2 HP air compressors, air dryer, piping, and controls; conversion of
two (2) existing lagoons to two (2) storm water EQ basins , approximately 2.85 MG
each, piping, and controls;conversion of one existing lagoon to a 4.2 MG concentrate
EQ basin, piping, and controls; replacement of existing NPW pumps with lower capacity
