The 30-day public notice and comment period, for the NPDES draft permit NC0026000, has
expired. There were no comments to the subject draft permit.
Attached is the NPDES Permit No. NC0026000 issued March 27, 2000. This permit is issued
pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between North Carolina and the US Environmental Protection
Agency dated December 6, 1983.
Please take notice that this permit is not transferable. Part II, E.4. addresses the requirements
to be followed in case of change in ownership or control of this discharge. This permit does not
affect the legal requirement to obtain other permits which may be required by the Division of
Water Quality or permits required by the Division of Land Resources, Coastal Area
Management Act or any other Federal or Local government permit that may be required.
If any parts , measurement frequencies or sampling requirements contained in this permit
modification are unacceptable to you, you have the right to an adjudicatory hearing upon
written request within thirty (30) days following receipt of this letter. This request must be a
written petition conforming to chapter 150B of the North Carolina General Statutes, filed with
the Office of Administrative Hearings, 6714 Mail Service Center, Raleigh, North Carolina
27699-6714. Unless such demand is made, this decision shall be final and binding.

Tabor City WWTP
off 701 Bypass East of Tabor City
Columbus County
to receiving waters designated as Town Canal to Grissit Swamp in the Lumber River Basin in
accordance with effluent limitations, monitoring requirements, and other conditions set forth in
Parts I, II, III and IV hereof.
This permit shall become effective May 1, 2000.
This permit and authorization to discharge shall expire at midnight on August 31, 2004.

SUPPLEMENT TO PERMIT COVER SHEET
Town of Tabor City
is hereby authorized to:
1. Continue operation of an existing wastewater treatment system, consisting of an influent lift station,
mechanical bar screen, grit chamber, automatic flow measurement, dual extended aeration basins,
dual secondary clarifiers, settling pond, post aeration pond, chlorination facilities, aerobic sludge
digestion, and sludge drying beds located at the Tabor City Wastewater Treatment Plant, off US 701
Bypass, east of Tabor City, Columbus County and;
2. After submitting the Engineers Certificate to the Division of Water Quality, operate a tertiary filter
system and submersible aerator at the above referenced facility, and;
3. Discharge treated wastewater from said treatment works at the location specified on the attached
map through outfall 001 into the Town Canal to Grissit Swamp which is classified C-Swamp waters in
the Lumber River Basin.

5. The pH of the effluent shall not be Tess than 6.0 nor greater than 9.0 (on the standard units scale).
6. After collection of 24 samples (2/month) over the period of one year, the permittee may request that the
Division review the monitoring data for possible removal of these parameters from the permit.
7. The daily average dissolved oxygen effluent concentration shall not be Tess than 5.0 mg/l.
8. Fecal Coliform shall be calculated using the geometric mean, according to the procedure detailed in Part II.
Section A, Paragraph 9b.
9. Instream samples shall be collected three times per week during June 1 through September 30 and once per
week October 1 through May 31.
There shall be no discharge of floating solids or visible foam in other than trace amounts.

A. (2) CHRONIC TOXICITY PERMIT LIMIT (QRTRLY)
The effluent discharge shall at no time exhibit observable inhibition of reproduction or significant mortality to
Ceriodaphnia dubia at an effluent concentration of 90%.
The permit holder shall perform at a minimum, quarterly monitoring using test procedures outlined in the "North
Carolina Ceriodaphnia Chronic Effluent Bioassay Procedure," Revised February 1998, or subsequent versions or
"North Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure" (Revised -February 1998) or subsequent
versions. The tests will be performed during the months of March, June, September, and December. Effluent
sampling for this testing shall be performed at the NPDES permitted final effluent discharge below all treatment
processes.
If the test procedure performed as the first test of any single quarter results in a failure or ChV below the permit
limit, then multiple -concentration testing shall be performed at a minimum, in each of the two following months as
described in "North Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure" (Revised -February 1998) or
subsequent versions.
The chronic value for multiple concentration tests will be determined using the geometric mean of the highest
concentration having no detectable impairment of reproduction or survival and the lowest concentration that does
have a detectable impairment of reproduction or survival. The definition of "detectable impairment," collection
methods, exposure regimes, and further statistical methods arc specified in the "North Carolina Phase II Chronic
Whole Effluent Toxicity Test Procedure" (Revised -February 1998) or subsequent versions.
All toxicity testing results required as part of this permit condition will be entered on the Effluent Discharge
Monitoring Form (MR-1) for the months in which tests were performed, using the parameter code TGP3B for the
pass/fail results and THP3B for the Chronic Value. Additionally. DWQ Form AT-3 (original) is to be sent to the
following address:

Completed Aquatic Toxicity Test Forms shall be filed with the Environmental Sciences Branch no later than 30 days
after the end of the reporting period for which the report is made.
Test data shall be complete, accurate, include all supporting chemical/physical measurements and all
concentration/response data, and be certified by laboratory supervisor and ORC or approved designate signature.
Total residual chlorine of the effluent toxicity sample must be measured and reported if chlorine is employed for
disinfection of the waste stream.
Should there be no discharge of flow from the facility during a month in which toxicity monitoring is required, the
permittee will complete the information located at the top of the aquatic toxicity (AT) test form indicating the
facility name, permit number, pipe number, county, and the month/year of the report with the notation of "No Flow"
in the comment area of the form. The report shall be submitted to the Environmental Sciences Branch at the address
cited above.
Should the permittee fail to monitor during a month in which toxicity monitoring is required, monitoring will be
required during the following month.
Should any test data from this monitoring requirement or tests performed by the North Carolina Division of Water
Quality indicate potential impacts to the receiving stream, this permit may be re -opened and modified to include
alternate monitoring requirements or limits.
NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum control organism
survival, minimum control organism reproduction, and appropriate environmental controls, shall constitute an
invalid test and will require immediate follow-up testing to be completed no later than the last day of the month
following the month of the initial monitoring.

SUMMARY
The Tabor City wastewater treatment plant is located off of the 701 bypass, east of Tabor City in
Columbus County. Tabor City has a monthly average permitted flow of 1.1 MGD and maintains
one outfall (001). The facility discharges treated wastewater into the Town Canal to Grissit
Swamp. The receiving stream, Town Canal to Grissit Swamp, is a class C-swamp waters located
in subbasin 03-07-57, in the Lumber River Basin. Subbasin 03-07-57 is located in Southeastern
North Carolina on the South Carolina/North Carolina state line. The subbasin has 350.6 miles of
streams, all of which have been classed as `fully supporting but threatened'. Land cover in the
basin is primarily Forest/wetland and agriculture. The total area of the subbasin is 535 square
miles with 1 square mile of this total being water.
The facility is a class III facility and operates the following treatment units: influent lift station,
mechanical bar screen, manual bar screen, grit chamber, automatic flow measurement, dual
extended aeration basins, dual secondary clarifiers, settling ponds, flow equilibrated chlorination
facilities, aerobic sludge digestion, and sludge drying beds.
The facility has been experiencing compliance problems over the past 4 years. The primary
problem has been chronic toxicity. Over the past four years the facility has failed numerous
Pass/Fail chronic toxicity test (30% pass rate over four years). There has been no Toxicity
Reduction Evaluation conducted to date, however the town manager states that a THE is
scheduled to begin the first week in January.
Additionally, the facility has been granted funding from the Clean Water Trust fund for the
installation of tertiary filters. The ATC for this proposed project was issued in October 1999 and
construction is scheduled to be completed by April 2000.
The facility had issued plans and specifications to the state for possible additional funding to
relocate their outfall to the Waccamaw River. However, the funding for this project was not
granted and the town has abandoned the project.

INSTREAM MONITORING:
Currently the facility is required to monitor fecal coliform, dissolved oxygen, and conductivity
weekly upstream and downstream (2 locations). The upstream location is above the discharge at
US 701 and the two downstream locations are approximately 75 yards below the outfall and at
the Iron Hill Bridge.
PROPOSED CHANGES:
The monitoring frequencies for BOD, total suspended solids, ammonia, total residual chlorine, pH,
dissolved oxygen, fecal coliform, conductivity and in -stream temperature monitoring have increased to
three times per week. The increase in monitoring frequency was implemented in order to comply with
the 2B .0500 Rules for a class III facility.
Cyanide, aluminum, cadmium, chromium, lead, mercury, nickel, selenium, and zinc monitoring have
been included as a condition for the permit. The permit application, submitted by Tabor City, states that
the towns WWTP effluent has the potential to contain the above mentioned toxicants. However, there is
insufficient data to determine if there is a reasonable potential for these toxicant to be present in Tabor
City's effluent. Therefore, monitoring for these parameters has been included as a condition for this
permit (See Part I; A(1)).
The Supplement to Permit Cover Sheet Page includes a line item allowing operation of the tertiary filters
and post aeration units after the engineers Certificate has been received.
The standard toxicity language has been included in this permit on the special condition page A(3)

Permit File Review
An ATC was issued in October of 1999 for the construction of pump station, force main,
tertiary filters and post aeration.
The current permit which is due to expire on December 31, 1999 has water quality
limited parameters of BOD, ammonia, and dissolved oxygen.
The permit application states that the following parameters have been detected in the
effluent: ammonia, cyanide, aluminum, cadmium, chromium, lead, mercury, nickel,
selenium, and zinc. Of these parameters, none are monitored or limited in the current
permit except ammonia.
In July 1999, Mark McIntire conducted an I/I evaluation for the facility where it was
concluded that the facility was not experiencing I/I problems.

No permit modifications have been issued since the last renewal.
Wasteload Allocation Information
According to the 1994 WLA the discharge is into a zero flow stream. The summer and
winter stream flows are 7Q10 = 0 and the 30Q2 = 0. The average stream flow is 1.0 cfs
with a drainage area of 1.0 mi2.

Conversation with Dave Goodrich — recommends that talk with the region, aquatic
toxicity unit discuss what is being done and what to do.
12/21/99 — Phone conversation with Steve West. The facility has not started to construct
the tertiary filters. The Town had talked about moving the outfall to the Waccamaw
River. The region is not sure that this will help the facility to meet their limits. Steve
concurs that having the facility monitor for metals is probably a good idea. He confirmed
that the facility does not have a pretreatment program.
12/21/99 — Phone conversation with Mike Williams. The facility assessed 7 times for
chronic toxicity violations. The facility is not responsive to evaluating the source of the
toxicity problems. The Town has stated that they have hired an outside consultant to
conduct a TRE. However, to date the region is unaware of any TRE work being
conducted.
12/21/99 —Phone conversation with Kevin Bowden. The facility is having significant
chronic toxicity problems. The facility has stated that a TRE will be done however, work
has not yet started. Technical Assistance and certification has been in contact with the
operator to offer their assistance in trying to train the operator to properly operate the
facility.
12/21/99 — Phone conversation with Steve Bevington. The facility received grant money
from the Clean Water Management Trust Fund in 12/97 for construction of plant
improvements. The project duration when the money was granted 12 months. The town
notified the clean water management trust fund that design was not complete in April '98.
In May '99 the Town notified the clean water management trust fund that the design was
complete at that they needed to secure their permits. The application package for the
ATC was submitted June 1999, with the approval granted in October 1999.
12/21/99 — Phone conversation with Al Leonard (Town of Tabor City) — The relocation
of the outfall is not proceeding. The Town submitted plans and specs but the funding
was not granted. Therefore, the Town is no longer pursuing the relocation of the outfall.
The construction of the tertiary filters is to be completed by April 2000. The bid has just
been awarded though construction has not begun. The TRE is being conducted by David
Lee from Greensboro. The TRE work is scheduled to begin the first week in January.
12/22/99 — Conversation with Tom Poe. Since the town has a industry discharging to the
WWTP then it is possible that the town should have a pretreatment program. Steve
Amigone, Tom Poe and Steve West may visit the site in January and determine if indeed
a pretreatment program is required.
12/22/99 — Phone conversation with Mike Williams. The industry in question is Pen
Ventilator. Mike is trying to establish if indeed the facility is discharging to the WWTP
and if the town has a pretreatment program.
Conclusion:
The facility has had compliance problems with BOD, ammonia, and chronic toxicity.
The major problem by far has been chronic toxicity. Over the past four years the facility
has collected 30 samples for chronic toxicity analysis. Of the 30 analysis conducted, the
facility has passed 9 of the analysis. That is a 30% pass rate.
The outfall relocation may not solve the Town's toxicity problems. The reason for this is
that the 7Q10 for Waccamaw River is approximately 2 cfs this would reduce the IWC
from 100 % to 46 %. Even if the relocation allowed the effluent to pass toxicity because
of the lower IWC, the problem is not resolved. This action solves the problem by
dilution instead of finding the source of the toxicity and taking appropriate action to
remove the source prior to discharge.
The facility has one industry discharging to the WWTP according to the NPDES permit
application. It is suggested that the Division encourage the completion of the TRE and
investigate getting a pretreatment program in place for the town.
Specific recommendations for the permit renewal include:
Metals monitoring for the parameters listed in the application having the potential to be
present in the effluent.
Changing the frequency of monitoring for BOD, TSS, NH3-N, TRC, pH, DO, Fecal
Coliform, conductivity, and Temperature to 3/week in order to be consistent with the
2B.0500 rules.
It is not recommended that the conductivity and fecal coliform be removed from the
permit according to the instream monitoring policy. The reason for this recommendation
is due to the unresolved issue surrounding the industrial discharger which discharges to
the WWTP.

Tabor City Wastewater Treatment Plant
Sludge Handling Plan
The Tabor City Wastewater Treatment Plant wastes solids into an aerobic digester. Lime
will be added to the aerobic digester prior to land application. These residual solids will
then be land applied according to the conditions spelled out in Permit No. WQ0001936
(attached).
