Name/Ownership Form Submittal for NCG050000
Unfortunately we cannot process the Permit Name/Ownership Change Request submitted on 10/22/2021 any
further. A Stormwater Program staff member will be in touch with more information.
Comments during review: Contacted submitter regarding incorrect permit number and clarification on
requested change on 3/14/2022. No response. A new request with the specific COC number should be submitted
through the form below:

Please contact Bethany Georgoulias at bethany.georgoulias@ncdenr.gov with any questions.

Subject: Company Name Change Request
Importance: High
Good afternoon,
We are working through some backlogged name change requests, and we had two submittals that listed the permit
number only as "NCG050000" (that's the general permit, and not the specific COC).
I need to confirm the following:
Requests are for NCG050304 (Facility Name "Salisbury Division") and ??? (I cannot find another permit number for CSP
Composites) — please advise.
This is a company name change only (not an ownership change)
CSP Composites LLC 4 TEIJIN Automotive Technologies North Carolina Composites, LLC
Facility name should also be revised as:
Salisbury Division 4 TEIJIN Automotive Technologies North Carolina Composites, LLC
Or were these two requests for the same site?
Thank you,
Bethany Georgoulias (she/her)
