I corresponded with you last year about this name/ownership change. We are catching up on requests because our
Program Administrative Assistant position has been vacant since October, and there has been a substantial delay in
reviewing them. We apologize for that. Unfortunately the uploaded attachment that came in with this request was not
adequate (attached). We need legal documentation of the transfer of ownership (see comments in the below email).

Unfortunately we cannot process the Permit Name/Ownership Change Request submitted on 12/8/2021 any
further. A Stormwater Program staff member will be in touch with more information.
Comments during review: Legal documentation of transfer of ownership (such as relevant pages of a contract
deed, or a bill of sale) is required for an ownership change request. A notification letter is not adequate. Please
submit a new request with appropriate supporting documentation using this form:
