EXISTING FACILITIES: MODIFICATION AND RENEWAL APPLICATIONS 1. Are the design, maintenance and operation of the treatment facilities adequate for the type of waste and disposal system? ® Yes or ❑ No If no, please explain: Description of existing facilities: Facilities consist of a two (2) bay truck wash, a 609,940 gallon vinyl -lined lagoon, a 194,764 gallon vinyl -lined recycle basin interconnected with the vinyl -lined lagoon, an irrigation pump tank with a 120 GPM pump, and a 2.55 acre spray irrigation area. Proposed flow: 9,500 GPD Current permitted flow: 9,500 GPD Explain anything observed during the site visit that needs to be addressed by the permit, or that may be important for the permit writer to know (i.e., equipment condition, function, maintenance, a change in facility ownership, etc.). Minimal washing is currently being accomplished (exterior of feed trucks), lagoons currently have about 3 feet of freeboard, and last use of irrigation system was to reduce the level of water due to rain. 3.
