Unfortunately we cannot process the Permit Name/Ownership Change Request submitted on 12/15/2021 any
further. A Stormwater Program staff member will be in touch with more information.
Comments during review: During the review, this appears to be an ownership change request. Please provide
the proper legal documentation of transfer of ownership (such as relevant pages of a contract deed, or a bill of

sale) is required for an ownership change request. Articles of incorporation are NOT sufficient for an ownership
change.
