Good afternoon, Mandi,
We are trying to get this permit sorted out that had been inactive. The current information is below (previous
owner). The Name/Owner change request submitted earlier this month only indicates a name change, which does not
appear correct (it does not even list the prior owner as the business below that held the permit). Also, there were no
documents uploaded to demonstrate the sale of the business.
Could you please have the new owners submit our current Name/Ownership request form below, with the appropriate
supporting documents?

In the case of an ownership change request, certifications must be signed by both the permit holder prior to
the change and the new applicant. For a name change request, the signed Permittee's Certification is
sufficient.
This completed application is required for both name change and/or ownership change requests.
North Carolina General Statute 143 - 215.6 b (i) provides that:
Any person who knowingly makes any false statement, representation, or certification in any application, record, report, plan, or other
document filed or required to be maintained under this Article or a rule implementing this Article; or who knowingly makes a false
statement of a material fact in a rulemaking proceeding or contested case under this Article; or who falsifies, tampers with, or
knowingly renders inaccurate any recording or monitoring device or method required to be operated or maintained under this Article
or rules of the [Environmental Management] Commission implementing this Article shall be guilty of a Class 2 misdemeanor which
may include a fine not to exceed ten thousand dollars ($10,000).

I apologize — I didn't remember the details of the previous emails (we've had to keep up with so much extra since we
lost so many staff back in the fall — I've lost track of many of them). Since the permit is expired, the fees will need to be
paid in order to renew it again, even if it's under new ownership. We will do our best to get to this ownership change
when we can. If you submitted it, it is still pending. I realize the permit COC is still listed under the old owner in our
system until then.
I am copying Aziza Selkane who will handle the billing side of this, for reference. When we do update the ownership
and contacts, who is the new billing contact? (Name, number, address, and email)
