The North Carolina Division of Water Resources conducted an inspection of the 732 Hidden Valley Road on
March 15, 2022. This inspection was conducted to verify that the facility is operating in compliance with the
conditions and limitations specified in NPDES WW Permit No. NCG550381. A summary of the findings and
comments noted during the inspection are provided in the enclosed copy of the inspection report.

The Compliance Evaluation inspection was conducted by Division of Water Resources staff from the Asheville
Regional Office. The following violation was noted during the inspection:

Lauren Armeni, with the Asheville Regional Office, conducted a Compliance Evaluation Inspection
(CEI) of the single-family wastewater disposal system that services 732 Hidden Valley Road on
March 15, 2022. This inspection was conducted to determine if the system is being operated and
maintained in compliance with General Permit no. NCG550000.
The system and property appeared well-maintained. In the 2016 CEI report, the effluent pipe
contained sediment and was damaged. During this inspection, the effluent pipe was easy to locate
and was no longer clogged with sediment. Discharge was observed from the effluent pipe during the
inspection; therefore, annual sampling is required. Annual sampling records were not provided. See
the attached General Permit No. NCG550000, Part I. Table C.(1.) Effluent Limitations and Monitoring
Requirements - Discharges to Freshwater (Class B and C waters only) for sampling requirements. A
list of certified laboratories has also been attached.
The following violation was noted during the inspection:
1. Effluent Sampling: The effluent pipe was discharging at the time of the inspection and annual
sampling has not been conducted. Annual sampling must be conducted if the system is discharging
to remain in compliance with General Permit No. NCG550000.
