Enclosed please find a copy of the Compliance Evaluation Inspection Report for the inspection
conducted on April 20, 2016 at your single family residence wastewater treatment system at 16
South Turkey Creek Road. Please read this report carefully and take note of any comments that
are listed. Please locate the discharge pipe and clearly mark the location for future reference.

Inspection performed by R. Topolski. Inspection entry in BIMS and CEI letter generated by G. Davidson
based on RT field notes (RT has left agency). Facility is in compliance with 05/12/2016 submittal of
septic tank pump receipt. Determination of a discharge pipe is not clear from field notes but indicates
"seam" found? Discharge pipe is to be located and clearly marked.

Solids, pH, D0, Sludge Judge, and other that are applicable?
Comment: Septic tank has two 6" PVC risers/caps. Some solids visible. Owner indicates it has been
7-8 years since last pumping. No dishcarge pipe observed but field notes indicate a "seam"
visible. Unclear regarding discharge pipe but owners highly responsive and have provided
pumping receipt on 05/12/2016.
