Enclosed please find a copy of the Compliance Evaluation Inspection form from the inspection conducted on
June 22, 2016. The Compliance Evaluation Inspection was conducted by Tim Fox and Kevin Mitchell of the
Asheville Regional Office. The facility is in compliance with permit NCG520024. Enclosed is a copy of the
NCG52000 permit conditions. Please continue to maintain and operate the site as per permit requirements
placing close attention to bank stabilization efforts and turbidity conditions during operations. We did not
observe warning signs outlined in the permit. Please review conditions and install signage if applicable.

Tim Fox and Kevin Mitchell with ARO visted the site. The site was not active the day of the inspection.
Settling ponds appeared to be in working order and being maintained. Bank stabilization appeared
stable with some ground cover. No problems identified during the site inspection.
We did not observe waming signs posted along the river or semi annual sampling data as per the
permit.
ARO will follow-up with the signs and sampling data requirements. Photos of the site were taken and
are in the file.
